【摘要】：正患者女,27歲,孕6周,因"妊娠1個(gè)月發(fā)現(xiàn)右側(cè)腹部搏動(dòng)性包塊"入院。體格檢查:血壓132/66mmHg,右中腹部觸及搏動(dòng)性包塊,右腎動(dòng)脈聽(tīng)診區(qū)未聞及雜音。實(shí)驗(yàn)室檢查無(wú)異常發(fā)現(xiàn)。腹部增強(qiáng)CT:右腎動(dòng)脈一級(jí)分支處可見(jiàn)巨大瘤樣擴(kuò)張,動(dòng)脈期呈高強(qiáng)化,凸向腎實(shí)質(zhì)外,大小約8.4cm×6.1cm(圖1A)。CT診斷:右腎動(dòng)脈動(dòng)脈瘤(renal artery aneurysm,RAA)�；颊咝心I動(dòng)脈造影:右腎上極可見(jiàn)巨大瘤樣擴(kuò)張,其內(nèi)血流呈湍流,動(dòng)脈瘤開(kāi)口位于右腎動(dòng)脈主干前段動(dòng)脈分支,該動(dòng)脈明顯增粗與腎動(dòng)脈主干相仿(圖1B),術(shù)中以錐形束CT掃描證實(shí)RAA位置及供血?jiǎng)用}。造影后患者行動(dòng)脈瘤腔內(nèi)栓塞
[Abstract]:The patient was 27 years old and 6 weeks pregnant. She was admitted to hospital because of "right abdominal pulsatile mass found in the first month of gestation". Physical examination: blood pressure 132 / 66mm Hg, right middle abdomen touching pulsatile mass. The right renal artery auscultation area was not heard and murmured. Laboratory examination showed no abnormal findings. Abdominal enhanced CTS: giant tumor-like dilatation was observed at the first grade branch of the right renal artery with high enhancement at the arterial stage and protruding toward the parenchyma of the kidney. The size was about 8.4 cm 脳 6.1 cm (fig. 1A). Ct diagnosis: renal artery aneurysm of right renal artery aneurysm. Renal arteriography: giant tumor-like dilatation was seen in the upper pole of the right kidney, and the blood flow was turbulent. The aneurysm opening was located in the branches of the anterior segment of the main trunk of the right renal artery. The thickening of the artery was similar to that of the main renal artery (fig. 1BX). The position of RAA and the feeding artery were confirmed by conical beam CT scan during the operation. Endovascular embolization was performed in the patients after angiography.
