【摘要】：目的通過對某集團(tuán)煤礦接塵工人肺功能損傷的現(xiàn)狀和流行病學(xué)特征的分析,探討其影響因素,為保護(hù)接塵工人健康提供科學(xué)依據(jù)。方法收集2013年6月-2014年5月該集團(tuán)甲、乙、丙、丁、戊5個煤礦879名男性接塵工人的健康監(jiān)護(hù)資料,調(diào)查其人口統(tǒng)計(jì)學(xué)信息、職業(yè)史、疾病史、吸煙情況等,分析該集團(tuán)煤礦接塵工人的肺功能損傷程度及其影響因素。結(jié)果接塵工人用力肺活量(FVC)、1秒鐘用力呼氣量(FEV1.0)、1秒鐘用力呼氣率(FEV1.0%)、峰值呼氣流速(PEFR)、最大呼氣中期流量(MMF)、25%FVC時(shí)的流速/被測者的身高(V25/Ht)異常檢出率分別為12.97%、15.93%、19.68%、39.82%、41.41%、34.70%。檢出通氣功能異常者為220例。肺功能損傷類型以阻塞性為主(12.06%),混合性次之(7.62%),限制性最少(5.35%)。丙和戊礦接塵工人肺功能損傷程度較重,表現(xiàn)為丙礦FVC、FEV1.0、FEV1.0%和戊礦V25/Ht的總體均數(shù)下降最明顯。吸煙指數(shù)為0、≤200、200時(shí),FEV1.0%的異常檢出率分別為8.05%、15.52%、40.98%;年齡為30、30~、40~和50歲~時(shí),FEV1.0%的總體均數(shù)分別為85.18±5.70、81.55±9.79、79.37±10.19、75.28±10.18;接塵工齡為10、10~、20~、30 a~時(shí),FVC的異常檢出率分別為13.33%、14.71%、18.63%、29.41%。多因素分析表明,排除年齡、吸煙的混雜影響,工種、接塵工齡是肺通氣功能損傷的危險(xiǎn)因素。結(jié)論該集團(tuán)煤礦接塵工人肺功能損傷主要表現(xiàn)為阻塞性通氣功能障礙,肺通氣功能的改變與年齡、接塵工齡、工種及吸煙的關(guān)系密切相關(guān)。
[Abstract]:Objective through the analysis of the status and epidemiological characteristics of lung function damage in the dust exposed workers in a group coal mine, the influence factors were discussed to provide scientific basis for protecting the health of the dust exposed workers. Methods the health monitoring data of 879 male dust workers in the 5 coal mines of the group A, B, C, C, D and E in the June 2013 year of -2014 were collected to investigate the population of their population. Statistical information, career history, history of disease, smoking and so on, analyzed the lung function damage degree and its influencing factors in the coal mine workers. Results the workers exposed to the forced expiratory volume (FVC), 1 second forced expiratory volume (FEV1.0), 1 second forced expiratory rate (FEV1.0%), peak expiratory flow rate (PEFR), maximum expiratory flow (MMF), 25%FVC The abnormal detection rates of velocity / height (V25/Ht) were 12.97%, 15.93%, 19.68%, 39.82%, 41.41%, and 220 cases with abnormal ventilatory function in 34.70%.. The types of pulmonary function injury were mainly obstructive (12.06%), mixed (7.62%), and Limited (5.35%). The lung function damage of the workers exposed to C and E was heavy, showing FV C mine The total average number of C, FEV1.0, FEV1.0% and AML V25/Ht decreased most obviously. When the smoking index was 0, or less than 200200, the abnormal detection rates of FEV1.0% were 8.05%, 15.52%, 40.98% respectively. The total average number of FEV1.0% was 85.18 + 5.70,81.55 + 9.79,79.37 + 10.18, respectively, with the age of 30,30~, 40~ and 50 years old, respectively. The abnormal detection rates were 13.33%, 14.71% and 18.63% respectively. 29.41%. multiple factors analysis showed that the exclusion age, the mixed effect of smoking, the work age and the age of the dust were the risk factors for lung ventilation injury. Conclusion the lung function damage of the coal dust workers in this group is mainly obstructive ventilation dysfunction, the change of lung ventilation function and age, and the connection of lung ventilation function and age. The relationship between working age, work and smoking is closely related.
