從會話分析的角度研究中國醫(yī)療機構(gòu)的醫(yī)患風險交談

為了實現(xiàn)成功的醫(yī)療服務,醫(yī)生與患者彼此之間的相互交流顯得尤為重要。可以說,醫(yī)患交際在醫(yī)療服務中起著關(guān)鍵作用。目前,研究者們也越來越重視對醫(yī)患交際的研究。本文運用會話分析的手段對醫(yī)患交際的其中一個方面——醫(yī)療風險交談——進行探討。本次研究所選語料均出自于對中國山西省內(nèi)醫(yī)療機構(gòu)的醫(yī)生對患者的問診過程的錄音和轉(zhuǎn)寫。會話分析的研究對象是人們在交往活動中的談話,它能更加直觀和詳細的展現(xiàn)人與人之間的會話活動。會話分析的主要概念包括話輪轉(zhuǎn)換、話輪設(shè)計、序列結(jié)構(gòu)和話題組織等。經(jīng)過對中國醫(yī)療機構(gòu)中的醫(yī)患風險交談進行認真分析,本研究主要從以下三個方面進行了探討。首先,參照的標準不同,醫(yī)療風險交談可有不同的分類。如:根據(jù)交談中是否談到利益可將醫(yī)療風險分為指定風險、均衡風險、對比風險和風險差異;根據(jù)風險交談主題發(fā)起人的不同可將醫(yī)療風險交談劃分為由醫(yī)生發(fā)起的醫(yī)療風險交談和由患者發(fā)起的醫(yī)療風險交談;根據(jù)交談效果的不同又可將醫(yī)療風險交談劃分為產(chǎn)生積極效果的醫(yī)療風險交談和產(chǎn)生消極效果的醫(yī)療風險交談。其次,醫(yī)患交談中會出現(xiàn)就不同話題進行的交談,醫(yī)療風險交談作為其中的一個話題必然有其組織的規(guī)律,話題組織就包括有對話題提出...
