【摘要】：目的:調(diào)查西安市醫(yī)療機構(gòu)臨床藥學(xué)工作開展?fàn)顩r,以促進(jìn)臨床藥學(xué)工作的進(jìn)一步發(fā)展。方法:分級分類抽取西安市24家二級以上醫(yī)療機構(gòu),對其臨床藥師現(xiàn)場進(jìn)行問卷調(diào)查,內(nèi)容包括醫(yī)院基本信息、臨床藥師的基本情況、臨床藥學(xué)工作開展?fàn)顩r。結(jié)果:被調(diào)研醫(yī)療機構(gòu)全職臨床藥師占60.2%,平均每位臨床藥師服務(wù)床位數(shù)為133.1±53.8(床/人)。各醫(yī)療機構(gòu)中主要開展的臨床藥學(xué)工作有用藥干預(yù)(95.8%)、藥學(xué)咨詢(91.7%)、細(xì)菌耐藥監(jiān)測(41.7%)、治療藥物監(jiān)測(20.8%)。臨床藥師進(jìn)行了查房(96.6%)、藥品不良反應(yīng)監(jiān)測(95.8%)、合理用藥培訓(xùn)(84.8%)、患者用藥教育(80.5%)、重點患者藥學(xué)監(jiān)護(78.0%)、特殊管理藥品藥學(xué)監(jiān)護(47.5%)、實習(xí)生和進(jìn)修人員培訓(xùn)(61.9%)、疑難危重患者會診(56.8%)、科研工作(42.4%)等臨床藥學(xué)工作。每天藥學(xué)服務(wù)時間多為1~2 h(68.6%的臨床藥師)。三級醫(yī)院、二級醫(yī)院每日查房頻次大于等于一次的分別為34.1%,3.5%。參與會診的臨床藥師有67.1%認(rèn)為"醫(yī)師會參考臨床藥師提供的用藥信息"。3.4%的臨床藥師認(rèn)為"ADR監(jiān)測工作開展很好且臨床很支持"。三級醫(yī)院臨床藥師更重視查房、ADR監(jiān)測和科研工作(P0.05),二級醫(yī)院臨床藥師與護士溝通較多(P0.05)。結(jié)論:西安市各級醫(yī)療機構(gòu)臨床藥學(xué)工作已經(jīng)普遍開展,但開展情況有差異,三級醫(yī)院查房頻次、ADR監(jiān)測和科研工作重視程度較高,二級醫(yī)院與護士溝通較多。
[Abstract]:Objective: to investigate the development of clinical pharmacy in Xi'an medical institutions in order to promote the further development of clinical pharmacy. Methods: 24 medical institutions above grade II in Xi'an were selected and investigated by questionnaire, including the basic information of hospital, the basic situation of clinical pharmacists and the development of clinical pharmacy. Results: the full-time clinical pharmacists in the investigated medical institutions accounted for 60.2%. The average number of beds served by each clinical pharmacist was 133.1 鹵53.8 (bed / person). The main clinical pharmaceutical work in various medical institutions was drug intervention (95.8%), pharmaceutical consultation (91.7%), bacterial drug resistance monitoring (41.7%) and therapeutic drug monitoring (20.8%). Clinical pharmacists carried out house rounds (96.6%), medicines adverse reactions monitoring (95.8%), reasonable drug use training (84.8%), patient drug use education (80.5%), key patients' pharmaceutical care (78.0%). Special management medicines pharmaceutical care (47.5%), interns and refresher training (61.9%), consultation of difficult and critical patients (56.8%), scientific research work (42.4%) and other clinical pharmaceutical work. The daily pharmaceutical care time was 1 脳 2 hours (68.6% of clinical pharmacists). The frequency of daily rounds in tertiary hospitals and secondary hospitals was 34.1% and 3.5% respectively. 67.1% of the clinical pharmacists who participated in the consultation thought that "doctors would refer to the information provided by clinical pharmacists". 3.4% of clinical pharmacists thought that "ADR monitoring was carried out well and clinically supported". Clinical pharmacists in tertiary hospitals pay more attention to house rounds, ADR monitoring and scientific research (P 0.05), and clinical pharmacists in secondary hospitals communicate more with nurses (P 0.05). Conclusion: the clinical pharmaceutical work of medical institutions at all levels in Xi'an has been widely carried out, but there are differences in the development. The frequency of hospital rounds in tertiary hospitals, ADR monitoring and scientific research pay more attention to the work, and the secondary hospitals communicate more with nurses.
