【摘要】：目的 :分析不同類型質(zhì)子泵抑制劑(Proton pump inhibitor,PPIs)預(yù)防應(yīng)激性潰瘍(Stress ulcer,SU)的臨床效果及藥物經(jīng)濟學(xué)比較。方法 :以我院2008年6月—2015年6月收治的652例接受PPIs預(yù)防SU的患者為研究對象,進行回顧性分析。按照患者PPIs藥物類型,將其分為奧美拉唑組(n=271)、蘭索拉唑組(n=228)及泮托拉唑組(n=153),比較各組患者SU發(fā)生率及成本—效果比等指標(biāo),探討最佳的SU預(yù)防藥物。結(jié)果 :奧美拉唑組SU發(fā)生率、死亡率顯著高于蘭索拉唑組、泮托拉唑組,差異有統(tǒng)計學(xué)意義(P0.05),蘭索拉唑組、泮托拉唑組SU發(fā)生率、死亡率比較,差異無統(tǒng)計學(xué)意義(P0.05)。各組患者不良反應(yīng)發(fā)生率比較,差異無統(tǒng)計學(xué)意義(P0.05)。奧美拉唑組成本—效果比(C/E)最低,蘭索拉唑組C/E最高,差異有統(tǒng)計學(xué)意義(P0.05)。結(jié)論 :PPIs在SU的預(yù)防中具有積極作用,其中,奧美拉唑預(yù)防SU成本最低,但效果有限,蘭索拉唑與泮托拉唑預(yù)防SU的效果相仿,而泮托拉唑成本更低,故建議將泮托拉唑作為SU的預(yù)防首選藥物。
[Abstract]:Aim: to analyze the clinical effect of proton pump inhibitor (Proton pump inhibitor,PPIs) in the prevention of stress ulcer (Stress ulcer,SU) and its pharmacoeconomic comparison. Methods: from June 2008 to June 2015, 652 patients who received PPIs to prevent SU were retrospectively analyzed. According to the drug types of PPIs, the patients were divided into three groups: omeprazole group (n = 271), lansoprazole group (n = 228) and pam Tora group (n = 153). The incidence of SU and the cost-effectiveness ratio were compared to explore the best preventive drugs for SU. Results: the incidence and mortality of SU in omeprazole group were significantly higher than those in lansoprazole group and pansoprazole group (P0.05). The incidence and mortality of SU in lansoprazole group and pam Tora group were significantly higher than those in lansoprazole group and pansoprazole group (P0.05). The difference was not statistically significant (P0.05). There was no significant difference in the incidence of adverse reactions in each group (P0.05). The cost-effect ratio (C / E) of omeprazole group was the lowest, and that of lansoprazole group was the highest (P0.05). Conclusion: PPIs plays an active role in the prevention of SU. The cost of omeprazole is the lowest, but the effect is limited. The effect of lansoprazole and pansoprazole on preventing SU is similar, but the cost of pam Tora is lower. Therefore, it is suggested that pam Tora azole should be the first choice for the prevention of SU.
