【摘要】：目的:減少北京大學(xué)首鋼醫(yī)院(以下簡(jiǎn)稱"我院")門診�？漆t(yī)師跨科開(kāi)藥所致的處方環(huán)節(jié)用藥錯(cuò)誤(以下簡(jiǎn)稱"處方錯(cuò)誤"),保障門診患者用藥的安全性、有效性和經(jīng)濟(jì)性。方法:采用回顧性研究方法,對(duì)我院2013年處方點(diǎn)評(píng)中發(fā)現(xiàn)的247例處方錯(cuò)誤進(jìn)行統(tǒng)計(jì)、分析。結(jié)果:247例用處方錯(cuò)誤中,跨科開(kāi)藥現(xiàn)象非常普遍;處方錯(cuò)誤的分類中,以給藥頻次和單次給藥劑量錯(cuò)誤最為常見(jiàn),分別占23.9%和22.7%,其次是無(wú)適應(yīng)證或超適應(yīng)證用藥和重復(fù)給藥,各占13.8%;療程、藥品品種、給藥途徑、禁忌證和溶劑錯(cuò)誤共占總用藥錯(cuò)誤的19.0%。跨科開(kāi)藥導(dǎo)致的處方錯(cuò)誤中,較為常見(jiàn)的依次為禁忌證用藥、給藥數(shù)量、給藥時(shí)間、給藥頻次和重復(fù)給藥錯(cuò)誤。結(jié)論:跨科開(kāi)藥是導(dǎo)致處方錯(cuò)誤和患者傷害的重要原因,醫(yī)院應(yīng)采取措施減少跨科開(kāi)藥的發(fā)生。
[Abstract]:Objective: to reduce the prescription errors caused by prescribing drugs by outpatients in Shougang Hospital of Peking University (hereinafter referred to as "our Hospital"), and to ensure the safety of drug use in outpatients. Efficiency and economy. Methods: 247 cases of prescription errors found in our hospital in 2013 were analyzed retrospectively. Results: in 247 cases of prescription errors, the phenomenon of prescribing drugs across departments was very common. In the classification of prescription errors, the frequency of administration and single dose error were the most common, accounting for 23.9% and 22.7am respectively, followed by unindications or overindications and repeated administration, accounting for 13.8%, respectively. Course of treatment, drug varieties, ways of administration, contraindications and solvent errors accounted for 19.0% of the total drug errors. Among the prescription errors caused by cross-disciplinary prescriptions, the most common ones were contraindications, dosage, time, frequency and repeated administration. Conclusion: cross-disciplinary prescribing is an important cause of prescription errors and injury of patients. Hospitals should take measures to reduce the occurrence of cross-disciplinary prescriptions.
