某院門急診抗菌藥物處方點評效果評價

【摘要】:目的:為門診抗菌藥物臨床合理使用提供參考。方法:采用回顧性分析方法,隨機抽取某院2013-2014年門、急診抗菌藥物處方進行統(tǒng)計、分析,并采用限定日劑量法對抗菌藥物的使用頻度(DDDs)進行分析。結果:通過處方點評,2013-2014年,該院抗菌藥物使用率從13.8%下降至11.9%;抗菌藥物處方不合理率從2.7%下降至1.2%。兒科、急診科、泌尿科抗菌藥物使用率較高;感染科、腎內科、急診科人均藥品費用較高。2014年較2013年兒科抗菌藥物使用率大幅下降,維持在40%左右;2014年1-9月急診科抗菌藥物使用率低于2013年同期,2014年1-9月全院抗菌藥物使用率低于2013年同期,10-12月則略高于2013年同期。門診抗菌藥物以單一用藥為主,2013、2014年占所有抗菌藥物處方比例分別為91.50%、90.77%,二聯(lián)用藥分別占8.44%、9.11%。2013-2014年DDDs排名前3位的抗菌藥物均為羅紅霉素、頭孢呋辛和頭孢克肟。結論:該院抗菌藥物的使用符合《2013年全國抗菌藥物臨床應用專項整治活動方案》要求。通過處方點評,該院門診抗菌藥物處方質量和合理使用的各項指標得到很大改善,但個別科室還存在抗菌藥物使用率偏高的情況;仍存在使用不合理現(xiàn)象。
[Abstract]:Objective: to provide reference for rational use of antibiotics in outpatient clinic. Methods: the prescriptions of antimicrobial drugs were selected randomly from a hospital in 2013-2014 and analyzed by retrospective analysis. The frequency of antimicrobial agents was analyzed by the method of limited daily dose (DDDs). Results: from 2013 to 2014, the utilization rate of antimicrobial agents decreased from 13.8% to 11.9%, and the irrational rate of antimicrobial prescriptions decreased from 2.7% to 1.2%. Paediatrics, emergency department, urology department, antibiotics use rate is higher; infection department, kidney medicine department, emergency department per capita drug cost is higher. 2014 compared with 2013 paediatric antimicrobial drug use rate drop sharply, The use rate of antimicrobial drugs in emergency department was lower than in the same period of 2013, and the utilization rate of antibiotics in the whole hospital was lower than that in the same period of October and December of 2013, which was slightly higher than that in the same period of 2013. In 2014, the proportion of antimicrobial drugs in all prescriptions was 91.50 and 9.11 respectively, and the two drugs accounted for 8.449.11.The top 3 antibiotics in the DDDs in 2013-2014 were roxithromycin, cefuroxime and cefixime, respectively, in 2013 and 2014, the proportion of antibiotics was 91.50 and 8.449.11. the top three antimicrobials in DDDs in 2013-2014 were roxithromycin, cefuroxime and cefixime. Conclusion: the use of antimicrobial drugs in our hospital is in line with the requirements of the 2013 National Program for the treatment of Antimicrobial Clinical Application. Through prescription evaluation, the quality of antibiotics prescription and the indexes of rational use of antimicrobial drugs in outpatient department of our hospital have been greatly improved, but the situation of high utilization rate of antimicrobial drugs still exists in some departments, and there is still unreasonable phenomenon in the use of antimicrobial drugs.
