沙利度胺對雙膦酸鹽相關(guān)頜骨壞死的影響

【摘要】:目的研究沙利度胺對雙膦酸鹽相關(guān)頜骨壞死的影響。方法 36只大鼠隨機(jī)分成A、B、C組,分別接受生理鹽水、唑來膦酸、唑來膦酸+沙利度胺治療。用藥3周后,拔除大鼠左上頜第1磨牙。拔牙后4、8周,收獲標(biāo)本,評價(jià)頜骨壞死、微血管生成及細(xì)胞凋亡情況。結(jié)果拔牙后4、8周,大鼠上頜骨拔牙創(chuàng)處均無死骨裸露,但B組、C組部分標(biāo)本可見一些小的瘺管。組織學(xué)檢查顯示,A組標(biāo)本未見死骨,而B組、C組在拔牙窩周圍可見小塊死骨。B組、C組拔牙窩周圍區(qū)域空骨陷窩百分比和死骨面積顯著多于A組(P0.01),而骨陷窩密度則顯著下降。B組、C組微血管密度較A組也顯著降低,在4周時(shí)分別下降了和25.87%和55.27%(P0.01),在8周時(shí)分別下降了45.62%和72.84%(P0.01);凋亡細(xì)胞數(shù)則在4周時(shí)分別增長了54.80%和87.89%(P0.01),在8周分別增長了208.08%和250.58%(P0.01)。結(jié)論沙利度胺加重了唑來膦酸誘發(fā)的早期階段的頜骨壞死;沙利度胺和唑來膦酸對頜骨壞死的作用與微血管生成抑制有關(guān)。
[Abstract]:Objective to study the effect of thalidomide on bisphosphonate-associated osteonecrosis of jaws. Methods Thirty-six rats were randomly divided into two groups: group A, B, C, treated with normal saline and thalidomide zoledronate, respectively. After 3 weeks, the left maxillary first molar was extracted. Eight weeks after tooth extraction, specimens were harvested to evaluate jaw necrosis, microangiogenesis and apoptosis. Results No dead bone was exposed at the site of maxillary extraction in rats 4 weeks after tooth extraction, but some small fistula was found in some specimens of group B and C. Histological examination showed that no dead bone was found in group A. The percentage of empty bone lacunae and dead bone area in group C were significantly higher than those in group A (P0.01), while the density of bone lacunae in group C was significantly lower than that in group A (P0.01). The microvessel density of group C was significantly lower than that of group A (P 0.01). The number of apoptotic cells decreased by 54.80% and 87.89% (P0.01) at 4 weeks, and by 208.08% and 250.58% (P0.01) at 8 weeks, respectively, and decreased by 45.62% and 72.84% (P0.01) at week 4 and by 25.87% and 55.27% (P0.01) respectively at week 4, and by 45.62% and 72.84% (P0.01) at week 8, respectively, and the number of apoptotic cells increased by 54.80% and 87.89% (P0.01) respectively. Conclusion thalidomide exacerbates the early stage of jaw necrosis induced by zoledronic acid, and the effects of thalidomide and zoledronic acid on jaw necrosis are related to the inhibition of microangiogenesis.
