【摘要】：目的:為相關(guān)部門制定醫(yī)院藥事服務(wù)收費(fèi)標(biāo)準(zhǔn)提供參考依據(jù)。方法:對(duì)某省10家"三甲"醫(yī)院藥學(xué)部2013年的人員收入情況以及門急診調(diào)劑服務(wù)量、各項(xiàng)成本支出情況等進(jìn)行問卷調(diào)查,在此基礎(chǔ)上統(tǒng)計(jì)和計(jì)算調(diào)劑服務(wù)總成本和單位成本,并對(duì)其影響因素進(jìn)行分析。結(jié)果:共發(fā)放問卷10份,回收有效問卷10份,有效回收率100%。10家受訪醫(yī)院除1家腫瘤專科醫(yī)院外,門、急診就診人次均在35萬人次以上,處方數(shù)均在30萬張以上,平均每張?zhí)幏郊s有3個(gè)藥品條目。受訪醫(yī)院藥房調(diào)劑服務(wù)單位成本約為平均每人次2.32元、每張?zhí)幏?.36元、每個(gè)藥品條目1.89元。相對(duì)于普通藥品,調(diào)劑需稀釋配置、拆零分裝、特殊管理的藥品所需的成本較高。敏感度分析表明,人力成本的變動(dòng)對(duì)于調(diào)劑服務(wù)成本的影響最大。結(jié)論:以藥品條目單位成本作為調(diào)劑服務(wù)成本的定價(jià)依據(jù)較為合理,人力成本因素是影響調(diào)劑成本的主要因素。
[Abstract]:Objective: to provide reference for the establishment of hospital drug service charge standard. Methods: a questionnaire survey was conducted on the personnel income, the quantity of outpatient and emergency dispensing services and the cost expenditure of the pharmacy department of 10 hospitals in a certain province in 2013, and the total cost and unit cost of the dispensing service were calculated and counted on the basis of the questionnaire. The influencing factors were analyzed. Results: a total of 10 questionnaires were sent out and 10 valid questionnaires were collected. The effective recovery rate of 100. 10 hospitals was more than 350000, and the number of prescriptions was more than 300000. On average, there are about 3 drug entries per prescription. The unit cost of dispensing service was about 2.32 yuan per person-time, 4.36 yuan per prescription and 1.89 yuan per drug item. Compared with the general medicine, the dispensing needs dilution and distribution, and the cost of special-managed drugs is higher. Sensitivity analysis shows that the change of human cost has the greatest influence on the cost of adjusting service. Conclusion: it is reasonable to take the unit cost of drug item as the pricing basis of adjusting service cost, and the factor of manpower cost is the main factor affecting the cost of dispensing.
