【摘要】：目的:為修訂和完善我國復(fù)方口服避孕藥(COCs)的使用標(biāo)準(zhǔn)和指南提供參考。方法:對世界衛(wèi)生組織(WHO)、美國與歐盟的COCs使用標(biāo)準(zhǔn)和指南進(jìn)行介紹和比較。結(jié)果與結(jié)論:WHO曾先后制定了《避孕方法選用的醫(yī)學(xué)標(biāo)準(zhǔn)》(MEC)、《避孕方法使用的選擇性實(shí)用建議》(SPR)、《避孕節(jié)育知情選擇咨詢指南》和《計(jì)劃生育服務(wù)提供者手冊》等4部關(guān)于COCs的使用標(biāo)準(zhǔn)和指南。其中,MEC按照使用者的具體情況進(jìn)行了適用、慎用、相對禁忌證、絕對禁忌證等分級;SPR主要介紹了如何安全、有效地使用各種避孕方法的具體內(nèi)容;《避孕節(jié)育知情選擇咨詢指南》使用導(dǎo)讀卡的模式,可快速查詢避孕節(jié)育臨床知情選擇中的具體方法和步驟;《計(jì)劃生育服務(wù)提供者手冊》可為在一線工作的計(jì)劃生育服務(wù)提供者進(jìn)行實(shí)際的工作指導(dǎo)。美國在WHO MEC和WHO SPR的基礎(chǔ)上,對個別項(xiàng)目進(jìn)行了修改和補(bǔ)充,包括對母乳喂養(yǎng)者、COCs對復(fù)發(fā)深靜脈血栓/肺栓塞風(fēng)險(xiǎn)的分級等。歐盟則早于WHO及美國制定了COCs的使用標(biāo)準(zhǔn)和指南,并不斷根據(jù)最新的循證醫(yī)學(xué)證據(jù)對COCs的關(guān)鍵風(fēng)險(xiǎn)點(diǎn)和特定品種進(jìn)行了提示和更新,包括重新評估使用COCs后的血栓栓塞風(fēng)險(xiǎn)和對達(dá)英35#174;及其仿制藥的特別安全提示等。我國應(yīng)基于WHO、美國與歐盟的COCs使用的最佳證據(jù),結(jié)合國情,為廣大使用者和醫(yī)護(hù)人員提供最新的、明確的COCs使用標(biāo)準(zhǔn)和指南。
[Abstract]:Objective: to provide a reference for the revision and improvement of the standards and guidelines for the use of Chinese compound contraceptive (COCs). Methods: the COCs standards and guidelines of WHO (WHO), USA and EU were introduced and compared. Results and conclusion: who has worked out four parts of "the Medical Standard of contraceptive method selection", "(MEC)," selective practical advice on contraceptive method use "," (SPR), "Counseling Guide to informed Choice of contraception" and "Manual of Family Planning Service providers", respectively. Standards and guidelines for the use of COCs. According to the specific situation of the user, the MEC is used carefully, the relative contraindication, the absolute contraindication and so on are classified as SPR, which mainly introduces how to be safe. Effective use of the specific content of various contraceptive methods; the guide to informed choice of contraception; the model for the use of reading cards, The detailed methods and steps in the clinical informed choice of contraception and birth control can be quickly queried, and the Manual of Family Planning Service providers can provide practical guidance for family planning service providers working in the front line. On the basis of WHO MEC and WHO SPR, individual projects have been modified and supplemented in the United States, including the classification of recurrent deep venous thrombosis / pulmonary embolism risk in breast-feeding patients. The European Union developed standards and guidelines for the use of COCs earlier than WHO and the United States, and constantly prompted and updated key risk points and specific types of COCs based on the latest evidence-based medical evidence. These include reassessment of thromboembolic risk after COCs use and special safety tips for Dafin 35 #174 and its generic drugs. Based on the best evidence of the use of COCs in the United States and the European Union, our country should provide the most up-to-date and clear standards and guidelines for the use of COCs for the vast number of users and medical personnel in combination with the national conditions.
