【摘要】：目的:了解某院泌尿外科門診超說明書用藥情況,為臨床合理用藥提供科學(xué)依據(jù),促進(jìn)臨床合理安全用藥。方法:收集某院自2014年1月1日-2016年1月1日泌尿外科門診處方,遴選出全部不符合藥品說明書所規(guī)定的用藥適應(yīng)證,劑量以及頻率的處方,進(jìn)行相關(guān)統(tǒng)計分析。對照國內(nèi)外相關(guān)指南、文獻(xiàn)報道分析超說明書用藥的合理性。結(jié)果:在所抽取的21 485張泌尿外科門診處方中,有524張?zhí)幏?2.4%)涉及超說明書用藥,共涉及藥品9種,均屬于超適應(yīng)證用藥范疇,未出現(xiàn)超劑量、超用藥頻次以及超療程用藥的情況。超適應(yīng)證用藥共7項,均有相關(guān)證據(jù)支持,其中2項符合國外內(nèi)指南用法;5項有相關(guān)的文獻(xiàn)報道支持。在所有超說明書用藥的處方中,α1-受體阻滯劑(坦索羅辛緩釋膠囊、多沙唑嗪緩釋片)所占比例最大(55.9%),所涉及的用藥適應(yīng)證是輸尿管下段結(jié)石。結(jié)論:該院的泌尿外科門診常見超說明書用藥均有相關(guān)證據(jù)支持,使用基本合理,但證據(jù)質(zhì)量存在差異。在臨床實際工作中,臨床藥師應(yīng)與處方醫(yī)師充分協(xié)作,權(quán)衡超說明書用藥的利弊,提高臨床合理用藥水平以及處方的合理性。
[Abstract]:Objective: to investigate the use of drugs in Urology outpatient department in order to provide scientific basis for rational use of drugs in clinic and promote rational and safe use of drugs in clinic. Methods: from January 1, 2014 to January 1, 2016, the outpatient prescriptions of urology in a hospital were collected, and the indications, doses and frequencies of drugs that did not conform to the drug instructions were selected for statistical analysis. Compared with domestic and foreign related guidelines, literature reports analyzed the rationality of drug use. Results: of the 21,485 urological outpatient prescriptions, 524 prescriptions (2.4%) were involved in superprescriptions, 9 kinds of drugs were involved, all of them belonged to the category of over-indications, and no overdose was found. The frequency of supermedication and the situation of drug use over the course of treatment. There were 7 overindications, all of which were supported by relevant evidence, of which 2 were in accordance with internal guidelines and 5 were supported by relevant literature reports. The 偽 1-receptor blocker (tansoloxin sustained-release capsule, doxazosin sustained-release tablet) accounted for the largest proportion (55.9%) among all the prescriptions of hyper-herbalism, and the indication involved was lower ureteral calculi. Conclusion: the common superprescriptions of Urology outpatient in our hospital are supported by relevant evidence, and the use is reasonable, but the quality of evidence is different. In clinical practice, clinical pharmacists should fully cooperate with prescription physicians to weigh the advantages and disadvantages of over-description of drugs and to improve the level of rational use of drugs in clinic and the rationality of prescriptions.
