【摘要】：目的:為靜脈藥物的安全、合理使用提供參考。方法:對(duì)我院靜脈藥物配置中心2009-2013年攔截的不合理處方進(jìn)行統(tǒng)計(jì)分析。結(jié)果:2009-2013年不合理處方5 802組,占總配置處方數(shù)量的0.12%;按原因排名前3位依次為溶劑用量不當(dāng)、藥品超量和溶劑選擇不當(dāng);按藥品類(lèi)別統(tǒng)計(jì)依次為抗菌藥物、抗腫瘤藥、普通藥品與腸外營(yíng)養(yǎng)液。在藥師的干預(yù)下,靜脈藥物不合理處方發(fā)生率從2009年的0.169 4%下降至2013年的0.082 8%;下降幅度最大的為用法錯(cuò)誤,其后依次為藥品超量、頻次錯(cuò)誤、醫(yī)囑開(kāi)立錯(cuò)誤、溶劑用量不當(dāng)、配伍禁忌以及溶劑選擇不當(dāng)。結(jié)論:藥師進(jìn)行用藥干預(yù)可有效降低不合理處方,顯著提高靜脈藥物的合理用藥水平。
[Abstract]:Objective: to provide reference for the safe and rational use of intravenous drugs. Methods: the irrational prescriptions intercepted by intravenous Drug allocation Center in our hospital from 2009 to 2013 were statistically analyzed. Results 5 802 unreasonable prescriptions were made from 2009 to 2013, which accounted for 0.12% of the total number of prescriptions; the top three reasons were improper solvent consumption, excessive dosage of drugs and improper selection of solvents; according to drug category statistics, antimicrobial drugs, antitumor drugs, General medicine and parenteral nutrition solution. Under the intervention of pharmacists, the incidence of irrational prescriptions of intravenous drugs decreased from 0.169 4% in 2009 to 0.082 8% in 2013. The biggest decrease was in usage errors, followed by overdose, frequency errors, wrong prescriptions and improper solvent consumption. Incompatibility and improper solvent selection. Conclusion: pharmacist intervention can effectively reduce irrational prescriptions and significantly improve the level of rational use of intravenous drugs.
