【摘要】：目的:為強(qiáng)化醫(yī)院超說(shuō)明書(shū)劑量用藥管理提供參考。方法:運(yùn)用PDCA循環(huán)管理法,在計(jì)劃階段調(diào)查我院內(nèi)分泌科2012年7月前列地爾注射液(Lipo-PGE1)超說(shuō)明書(shū)劑量使用現(xiàn)狀,發(fā)現(xiàn)問(wèn)題,尋找原因,制訂干預(yù)目標(biāo)和措施;在實(shí)施階段進(jìn)行干預(yù),在檢查階段定期審核醫(yī)囑,并在處理階段抽查2013年同期數(shù)據(jù),分析干預(yù)效果,將干預(yù)流程標(biāo)準(zhǔn)化。結(jié)果:PDCA循環(huán)干預(yù)前共分析用藥醫(yī)囑943條、患者126例次,分別按用藥患者數(shù)、用藥醫(yī)囑數(shù)和藥品使用量計(jì)算,超說(shuō)明書(shū)劑量用藥發(fā)生率分別為39.68%、31.50%和47.90%;經(jīng)過(guò)一個(gè)PDCA循環(huán)干預(yù)后,調(diào)查2013年同期用藥醫(yī)囑414條、患者73例次,結(jié)果超說(shuō)明書(shū)劑量用藥發(fā)生率為0。干預(yù)前后Lipo-PGE1的實(shí)際處方日劑量(PDD)由13.15μg下降到說(shuō)明書(shū)規(guī)定的范圍內(nèi)(10.00μg),下降率23.95%;藥物使用強(qiáng)度(DUD)由59.82下降到20.07,下降率66.45%。干預(yù)前后Lipo-PGE1的超說(shuō)明書(shū)劑量用藥發(fā)生率、PDD、DUD比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。本次干預(yù)達(dá)到了預(yù)期的目標(biāo)值。結(jié)論:PDCA循環(huán)管理法以其條理清晰的工作思路和持續(xù)改進(jìn)的循環(huán)方法,為醫(yī)院超說(shuō)明書(shū)劑量用藥管理提供了一個(gè)可行的途徑。
[Abstract]:Objective: to provide reference for strengthening the management of overdose medication in hospital. Methods: by using PDCA circulation management method, we investigated the present situation of the use of Lipo-PGE1 overdose in the Endocrinology Department in July 2012, found the problems, looked for the causes, and worked out the intervention goals and measures. The intervention is carried out in the implementation stage, the doctor's order is reviewed regularly in the inspection stage, and the data of the same period in 2013 are checked at the processing stage, the intervention effect is analyzed, and the intervention process is standardized. Results before the intervention, 943 prescriptions were analyzed, 126 cases of patients, calculated according to the number of patients, the number of medical orders and the amount of drugs used, the incidence of overdose drug use was 39.68% and 47.90% respectively, after a PDCA cycle intervention, the incidence of drug use was 39.68% and 47.90%, respectively, after a PDCA cycle intervention, the incidence of drug use was 39.68% and 47.90% respectively. Conclusion with its clear working idea and continuously improving circulation method, the WPDCA circulation management method provides a feasible way for the management of overdose medication in hospital.
