【摘要】：目的:分析基本藥物制度實(shí)施對(duì)欠發(fā)達(dá)地區(qū)基層醫(yī)療機(jī)構(gòu)產(chǎn)生的影響。方法:通過發(fā)放機(jī)構(gòu)調(diào)查表收集蘇北某縣17家鄉(xiāng)鎮(zhèn)衛(wèi)生院2013~2015年期間相關(guān)數(shù)據(jù)資料,并結(jié)合人物訪談進(jìn)行綜合分析。結(jié)果:從2013年到2015年,樣本縣鄉(xiāng)鎮(zhèn)衛(wèi)生院配備的基本藥物品種數(shù)由872種降至666種,其中國(guó)家基本藥物占比由56.77%降至54.50%;總衛(wèi)生服務(wù)量有所增加,其中門診和住院服務(wù)量年均增長(zhǎng)率分別為46.82%和-23.86%;收入結(jié)構(gòu)中主要來源是醫(yī)療收入,占比48.20%~54.20%,藥占比和財(cái)政補(bǔ)助占比21.30%~23.00%。結(jié)論:推行基本藥物制度過程中還存在國(guó)家基本藥物配備使用欠缺主導(dǎo)地位、基本藥物制度實(shí)施對(duì)住院服務(wù)有一定的抑制作用和基本藥物零差率銷售背景下財(cái)政補(bǔ)助水平偏低的問題。
[Abstract]:Objective: to analyze the effect of the implementation of essential drugs system on primary medical institutions in underdeveloped areas. Methods: the relevant data of 17 township health centers from 2013 to 2015 in a county in northern Jiangsu were collected and analyzed by means of distribution organization questionnaire. Results: from 2013 to 2015, the number of essential drugs provided in the sample county township health centers decreased from 872 to 666, in which the proportion of national essential drugs decreased from 56.77% to 54.50%, and the total amount of health services increased. The average annual growth rate of outpatient service and inpatient service was 46.82% and -23.86%, respectively, and the main source of income was medical income, accounting for 48.20% and 54.20%, and the proportion of medicine and financial subsidy was 21.30% and 23.00%, respectively. Conclusion: in the process of implementing the essential drug system, there is still a lack of dominant position in the use of essential drugs in the country. The implementation of essential drugs system has a certain inhibitory effect on hospital service and the problem of low level of financial subsidy under the background of zero difference rate of sale of essential drugs.
