【摘要】：目的:探討臨床藥師在消化科開展藥學服務(wù)和參與臨床實踐的模式和方法。方法:根據(jù)消化科患者及所使用藥物的特點,對標準化流程、抗菌藥物管理、藥品不良反應(yīng)(ADR)監(jiān)測、個體化用藥監(jiān)護、護理人員用藥安全指導等方面進行回顧性總結(jié)。結(jié)果:1例男性肝膿腫患者,帕尼培南調(diào)整為1 g,bid,患者用藥后4 d未出現(xiàn)體溫反跳,逐漸趨于平穩(wěn),治療后復查B超肝膿腫明顯縮小。1例女性肝膿腫伴腎功能重度損害患者,調(diào)整亞胺培南/西司他丁鈉0.25 g,tid,ivgtt抗感染治療10 d,CT顯示肝膿腫明顯縮小,且血肌酐下降至128μmol/L,遂將亞胺培南/西司他丁鈉改為0.5 g,tid,ivgtt,12 d后調(diào)整為頭孢吡肟1 g,bid,ivgtt繼續(xù)治療。18 d后,患者好轉(zhuǎn)出院。臨床藥師共填報消化科病區(qū)ADR 26例,其中與輸液相關(guān)16例,占61.54%;與抗生素相關(guān)12例,占46.15%;嚴重ADR 10例,其中新的嚴重ADR 3例;8例為院外已發(fā)生藥物性肝損傷。應(yīng)結(jié)合消化科疾病種類繁多、聯(lián)合用藥普遍、藥源性疾病發(fā)生率高等特點,找準切入點,為臨床提供適宜的藥學服務(wù)。
[Abstract]:Objective: to explore the mode and method for clinical pharmacists to carry out pharmaceutical care and participate in clinical practice in digestive department.Methods: according to the characteristics of the patients and the drugs used in digestive department, the standardization process, antimicrobial management, ADR monitoring, individualized drug monitoring and safety guidance of drug use by nursing staff were summarized retrospectively.Results in 1 male patient with liver abscess, Panipenem was adjusted to 1 g bidbid.There was no rebound of body temperature at 4 days after treatment, which tended to be stable.
