【摘要】：背景:隨著現(xiàn)代藥學(xué)模式的轉(zhuǎn)變,公眾對(duì)臨床用藥水平和藥學(xué)服務(wù)質(zhì)量提出了更高的要求。為此,建立以“患者為中心”的藥學(xué)服務(wù)模式,開(kāi)展以合理用藥為核心的臨床藥學(xué)工作,提供滲透于整個(gè)醫(yī)療衛(wèi)生保健過(guò)程中的藥學(xué)服務(wù),是現(xiàn)代醫(yī)學(xué)模式的必然要求,也是現(xiàn)代藥學(xué)的發(fā)展方向。目的:本研究在調(diào)查分析陜西省藥學(xué)人力資源現(xiàn)狀的基礎(chǔ)上,對(duì)藥學(xué)人力資源服務(wù)質(zhì)量進(jìn)行測(cè)量研究,為優(yōu)化醫(yī)療機(jī)構(gòu)藥學(xué)人力資源結(jié)構(gòu),提高藥學(xué)服務(wù)質(zhì)量,提出相應(yīng)的對(duì)策建議。方法:臨床工作者和藥學(xué)人員的樣本取自陜北榆林市、關(guān)中西安市和咸陽(yáng)市、陜南漢中市等多家樣本機(jī)構(gòu)共450名工作人員,迷你臨床演練的樣本選取29名藥學(xué)工作人員,并在陜西省7個(gè)地區(qū)選取居民樣本441名。采用文獻(xiàn)復(fù)習(xí)和描述性統(tǒng)計(jì)分析方法,測(cè)量藥學(xué)人員的專業(yè)知識(shí)技能水平測(cè)試得分基本情況、臨床用藥指導(dǎo)情況,處方合理用藥情況。結(jié)果:1.藥學(xué)人員專業(yè)知識(shí)技能水平測(cè)量情況:本次測(cè)試包含50道題目,涉及藥品的儲(chǔ)存與保管、藥品管理的特殊性、藥品的不良反應(yīng)等多個(gè)方面�？傮w來(lái)看,本次測(cè)試的正確率并不理想。不同性別、學(xué)歷、職稱、崗位的得分情況不同。2.臨床用藥指導(dǎo)測(cè)量情況:工齡15年以上的工作者對(duì)處方“四查十對(duì)”工作、不合理處方審核的工作、用法用量交代的工作、藥學(xué)面談技能等方面的各部分得分都明顯高于15年以下的工作者,說(shuō)明工作經(jīng)驗(yàn)豐富員工工作更熟練。3.處方合理用藥測(cè)量情況:不合理處方現(xiàn)象嚴(yán)重,藥師受重視程度低,以及沒(méi)有足夠的審方權(quán),一定程度上影響著合理用藥的水平。建議:1.增加臨床藥師配備,提高臨床藥師的數(shù)量和質(zhì)量2.加大藥學(xué)生培養(yǎng)力度,優(yōu)化藥學(xué)人才資源知識(shí)結(jié)構(gòu)3.提高對(duì)藥師的重視程度,給予藥師相應(yīng)的審方權(quán)4.醫(yī)院提高對(duì)用藥咨詢服務(wù)的費(fèi)用支持,建立相關(guān)的藥學(xué)咨詢服務(wù)信息化平臺(tái)5.加強(qiáng)對(duì)藥學(xué)工作人員臨床知識(shí)的定期或不定期培訓(xùn)6.規(guī)范處方,加快處方信息化建設(shè)步伐
[Abstract]:Background: with the change of modern pharmaceutical model, the public has put forward higher demands on the level of clinical drug use and the quality of pharmaceutical service. Therefore, a "patient-centered" pharmaceutical service model has been established. It is an inevitable requirement of modern medical model to carry out clinical pharmaceutical work with rational drug use as the core and to provide pharmaceutical services that permeate the whole process of medical care. Objective: based on the investigation and analysis of the present situation of pharmaceutical human resources in Shaanxi Province, the quality of pharmaceutical human resources service was measured and studied in order to optimize the structure of pharmaceutical human resources in medical institutions. Methods: the samples of clinical workers and pharmacists were collected from several sample organizations, such as Yulin City, Guanzhong Xi'an City and Xianyang City, and Hanzhong City, South Shaanxi Province. A sample of 29 pharmaceutical workers was selected from the mini clinical exercise and 441 from 7 regions of Shaanxi Province. Literature review and descriptive statistical analysis were used. Measurement of the professional knowledge and skill level of pharmaceutical personnel: basic score, clinical medication guidance, prescription rational use of drugs. Results: 1. Measurement of professional knowledge and skill level of pharmaceutical personnel: this test includes 50 questions, It involves the storage and custody of drugs, the particularity of drug management, the adverse reactions of drugs, etc. In general, the correct rate of this test is not ideal. The scores of posts are different .2.The measurement of clinical drug use guidance: the workers who have worked for more than 15 years have a job of "four checks and ten pairs" of prescriptions, the work of reviewing unreasonable prescriptions, and the work of explaining the usage of prescriptions. The scores of pharmaceutical interview skills were significantly higher than those of workers less than 15 years, indicating that the workers with rich working experience were more skillful in their work. 3.The rational prescription drug use measurement: unreasonable prescription phenomenon was serious, and pharmacists were less valued. And there is not enough power to review the prescription, which to some extent affects the level of rational use of drugs. It is suggested that: 1. Increase the availability of clinical pharmacists, improve the quantity and quality of clinical pharmacists, and increase the training of drug students. Optimizing the knowledge structure of Pharmacy Talent Resources 3.To improve the degree of attention to pharmacists and give the pharmacists the right to examine the prescriptions. 4. The hospitals should increase the cost support for drug use consulting services. 5. Strengthen the regular or irregular training on the clinical knowledge of pharmaceutical workers. 6. Standardize prescriptions and speed up the construction of prescription informatization
