【摘要】：目的:降低醫(yī)院靜脈藥物配置中心(PIVAS)中高危藥品的調(diào)配風(fēng)險(xiǎn)。方法:介紹我院PIVAS對高危藥品進(jìn)行風(fēng)險(xiǎn)控制的主要措施,包括成立高危藥品風(fēng)險(xiǎn)控制小組、建立高危藥品目錄、除配送成品外的PIVAS工作全由藥學(xué)專業(yè)人員承擔(dān)、高危藥品使用流程實(shí)現(xiàn)信息化管理環(huán)節(jié)并強(qiáng)化臨床藥師的參與等。通過統(tǒng)計(jì)近3年P(guān)IVAS發(fā)生的差錯(cuò)及不良事件評價(jià)管理效果并總結(jié)經(jīng)驗(yàn)。結(jié)果與結(jié)論:2012-2014年,我院PIVAS與高危藥品相關(guān)的差錯(cuò)發(fā)生率低(0.009‰、0.004‰、0.002‰)且逐年減少,由2012年的8件減少為2013年的3件、2014年的2件;其中加強(qiáng)核對、加強(qiáng)新進(jìn)人員的操作訓(xùn)練、PIVAS人員實(shí)現(xiàn)藥學(xué)專業(yè)化及建立高危藥品的警示標(biāo)識對風(fēng)險(xiǎn)控制具有重要作用。

Methods: the main measures of PIVAS risk control for high risk drugs in our hospital were introduced, including the establishment of high risk control group and the establishment of high risk drug catalogue.
