【摘要】：目的了解北京大學(xué)第三醫(yī)院藥源性過敏性休克(DIAS)和嚴(yán)重過敏樣反應(yīng)(DIAR)的致敏藥物類型及過敏反應(yīng)的發(fā)生情況(包括臨床表現(xiàn)、救治方案及預(yù)后等)。方法篩選北京大學(xué)第三醫(yī)院2004-01-01—2014-12-31上報(bào)國家藥品不良反應(yīng)監(jiān)測系統(tǒng)的DIAS和DIAR報(bào)告,對(duì)納入報(bào)告患者的一般情況、致敏藥物類型、臨床表現(xiàn)、治療方案及轉(zhuǎn)歸等資料進(jìn)行分析。結(jié)果共納入69份不良反應(yīng)報(bào)告,男女比例為1.00∶1.56,以速發(fā)型超敏反應(yīng)為主(85.5%),最常見的致敏藥物為抗菌藥物(45.5%),臨床癥狀主要表現(xiàn)為心血管系統(tǒng)(92.8%)、呼吸系統(tǒng)(58.0%)和皮膚黏膜系統(tǒng)(58.0%)受累。在DIAS和DIAR救治中,糖皮質(zhì)激素(58.0%)、補(bǔ)液治療(53.6%)以及抗過敏藥物治療(40.6%)最為常見。69份不良反應(yīng)報(bào)告中,經(jīng)救治后痊愈29例(42.0%),好轉(zhuǎn)37例(53.6%),死亡、住院治療、數(shù)據(jù)缺失各1例(1.4%)。結(jié)論 DIAS與DIAR占全部報(bào)告的2.5%(69/2761),由于其存在不可預(yù)測性,對(duì)患者危害較大,建立規(guī)范的救治及預(yù)防措施具有重要意義。
[Abstract]:Objective to investigate the types of drug sensitizers and the occurrence of allergic reactions (including clinical manifestations) in Dish and DIAR in the third Hospital of Peking University. Methods the DIAS and DIAR reports were selected from the third Hospital of Peking University from January 1 to 31, 2004 to report to the National adverse Drug reaction Surveillance system. The general situation of the patients included in the report, the types of sensitized drugs, and the clinical manifestations were analyzed. The data of treatment plan and outcome were analyzed. Results 69 ADR reports were included. The ratio of male to female was 1.00: 1.56, with a rapid hypersensitivity rate of 85.55.The most common allergen was the antimicrobial drug, 45.5%. The main clinical symptoms were the cardiovascular system (92.880%) and the respiratory system (58.0%) and the skin and mucosal system (58.0%). In the treatment of DIAS and DIAR, Of the 69 reports of adverse reactions, 29 cases were cured after treatment, 37 cases were cured and 53.6% were cured, 37 cases were cured, 53.6% died, and were hospitalized. Conclusion DIAS and DIAR account for 2.569 / 2761% of the total reported data. Because of their unpredictability, they are harmful to patients. It is of great significance to establish standard treatment and prevention measures.
