【摘要】：目的：本課題旨在借鑒國內外臨床抗菌藥物管理方面的經(jīng)驗，針對國內臨床抗菌藥物管理方面存在的問題，研究臨床抗菌藥物管理計算機網(wǎng)絡系統(tǒng)應用于臨床的效果。開發(fā)醫(yī)院臨床抗菌藥物用藥情況查詢和分析軟件，利用計算機網(wǎng)絡，快速檢索和統(tǒng)計全院抗菌藥品使用情況。通過分析臨床抗菌藥物的使用頻率以及抗菌藥物處方占比、聯(lián)合用抗菌藥頻率、抗菌藥物使用強度等指數(shù)評估醫(yī)院抗菌藥物管理水平。以期達到切實提高醫(yī)院抗菌藥物管理、最大限度地實現(xiàn)“以病人為中心”的醫(yī)療服務目的。所進行的具體應用有望使抗菌藥物管理水平能夠顯著提升，提高醫(yī)院抗菌藥物管理水平。 方法：通過調查、問詢，隨訪臨床專家，醫(yī)師及藥師等研究抗菌藥物使用現(xiàn)狀及原因；選取2013年7月至2014年6月期間，在某二級綜合醫(yī)院應用抗菌藥物管理系統(tǒng)后為實施組，同時在信息庫中抽調出2012年7月至2013年6月期間在該院未應用抗菌藥物管理系統(tǒng)前為對照組，兩組進行比較；統(tǒng)計分析采用SPSS17.0和Excel2007統(tǒng)計軟件進行數(shù)據(jù)分析。其中計量資料以均數(shù)和標準差（X2±S）表示，兩組之間的比較采用t檢驗；計數(shù)資料應用χ檢驗。差異的顯著性以P0.05為有統(tǒng)計學意義。 結果：信息化管理是醫(yī)院今后發(fā)展的必然趨勢，抗菌藥物管理系統(tǒng)（AMIS）的運用，，大大優(yōu)化了醫(yī)院業(yè)務流程，提升了醫(yī)院管理水平，建立了以患者為中心的服務模式，實現(xiàn)了良好的社會效益、經(jīng)濟效益、管理效益，達到了醫(yī)院信息化建設的根本目標。本研究顯示，應用抗菌藥物管理系統(tǒng)后，實施組的門診抗菌藥物處方數(shù)較對照組降低了6.43%，實施組的抗菌藥物使用人次占出院人次比例較對照組降低了4.11%，實施組的I類切口手術抗菌藥物使用率較對照組降低了4.8%，實施組的住院患者抗菌藥物使用強度（DDD值）較對照組降低了28.88，經(jīng)檢驗P值均0.05，具有統(tǒng)計學意義，提示兩組間有顯著差別。 結論：本文就抗菌藥物管理系統(tǒng)平臺的應用背景和抗菌藥物管理系統(tǒng)的價值和方式進行了總結概述，并闡述了通過實踐應用系統(tǒng)平臺監(jiān)測抗菌藥物管理的主要效果。通過比較分析，驗證了該項研究對抗菌藥物管理水平的推進和顯著提升。臨床抗菌藥物管理計算機網(wǎng)絡系統(tǒng)為臨床抗菌藥物使用提供了有效的指導和監(jiān)測手段�？咕幬锕芾硐到y(tǒng)已成為我院管理、臨床醫(yī)藥工作必不可少的工具，可為臨床抗菌藥物管理提供數(shù)據(jù)分析處理支持，提高臨床抗菌藥物管理水平，規(guī)范醫(yī)療行為，提高醫(yī)療質量。
[Abstract]:Objective: the purpose of this project is to draw lessons from the experience of clinical antimicrobial management at home and abroad and to solve the problems existing in the management of domestic clinical antimicrobial agents. To study the effect of clinical antimicrobial drug management computer network system applied in clinic, to develop the software for inquiry and analysis of clinical antimicrobial drug use in hospital, and to use computer network, By analyzing the frequency of use of clinical antimicrobials and the proportion of antimicrobial prescriptions, the frequency of combined use of antimicrobial drugs was analyzed. To evaluate the management level of antimicrobial drugs in hospitals with the index of antimicrobial use intensity, so as to improve the management of antimicrobial drugs in hospitals, It is expected that the management level of antimicrobial agents will be improved significantly and the management level of antimicrobial agents in hospitals will be improved. Methods: through investigation, inquiry, follow up of clinical experts, doctors and pharmacists, to study the current situation and reasons of antimicrobial drug use, and to select the application of antimicrobial management system in a level II general hospital between July 2013 and June 2014 as the implementation group. At the same time, from July 2012 to June 2013, the control group was selected as the control group before the use of antimicrobial drug management system in the hospital from July 2012 to June 2013, and the two groups were compared. The statistical analysis was carried out by SPSS17.0 and Excel2007 statistical software. The measurement data were expressed as mean and standard deviation X2 鹵S, the comparison between the two groups was t test, and the count data was analyzed by 蠂 ~ 2 test. The significance of the difference was P0.05. Results: information management is the inevitable trend of hospital development in the future. The application of antimicrobial drug management system Amis has greatly optimized the hospital business process, improved the hospital management level, and established a patient-centered service model. It has achieved good social benefits, economic benefits and management benefits, and achieved the fundamental goal of hospital information construction. This study shows that, after the application of antimicrobial drug management system, The number of antibiotics prescriptions in the outpatient department of the implementation group was 6.43 lower than that in the control group, and the proportion of antibiotics used in the treatment group was 4.11% lower than that in the control group. The utilization rate of antibiotics in the group I was lower than that in the control group. The DDD value of the inpatients in the implementation group was 28.88 lower than that of the control group, and the P value was 0.05, which was statistically significant. The results suggest that there is a significant difference between the two groups. Conclusion: in this paper, the application background of antimicrobial management system and the value and method of antimicrobial management system are summarized and summarized. The main effect of monitoring antimicrobial drug management through the platform of practical application system is expounded. The computer network system of clinical antimicrobial management provides effective guidance and monitoring means for clinical use of antimicrobial agents. It has become the management of our hospital. The essential tools of clinical medicine work can provide data analysis and processing support for clinical antimicrobial drug management, improve the management level of clinical antimicrobial agents, standardize medical behavior and improve medical quality.
