【摘要】：目的:了解我國(guó)腸外營(yíng)養(yǎng)處方中維生素的使用情況,為我國(guó)腸外營(yíng)養(yǎng)中維生素的合理使用提供參考。方法:以"腸外營(yíng)養(yǎng)"或"靜脈營(yíng)養(yǎng)"和"處方分析"或"合理用藥"或"醫(yī)囑分析"為關(guān)鍵詞,檢索中國(guó)知網(wǎng)和萬(wàn)方數(shù)據(jù)庫(kù)中2006-2015年報(bào)道的有關(guān)腸外營(yíng)養(yǎng)處方合理性分析文獻(xiàn),對(duì)納入文獻(xiàn)中維生素的使用情況進(jìn)行分析和總結(jié)。結(jié)果:共納入34篇文獻(xiàn),涉及的總處方數(shù)為158 874份,其中145 400份(91.52%)處方使用了維生素;有73 508份(46.27%)處方使用了12或13種維生素,53 388份(33.60%)處方被文獻(xiàn)作者認(rèn)為超量使用維生素,18 504份(11.65%)處方維生素添加不完全,1 663份(1.05%)處方未添加維生素。我國(guó)北部地區(qū)添加12或13種維生素的比例最高(52.00%)。"三甲"醫(yī)院添加12或13種維生素、文獻(xiàn)作者認(rèn)為超量使用維生素和水溶性維生素、脂溶性維生素均未完全添加的比例均低于"三甲"以下醫(yī)院。使用12或13種必需維生素比例由高到低的處方科室依次為腫瘤科(97.72%)、內(nèi)科(86.62%)、重癥監(jiān)護(hù)病房(77.99%)和外科(52.83%)。結(jié)論:我國(guó)腸外營(yíng)養(yǎng)處方中維生素規(guī)范使用的比例偏低,腸外營(yíng)養(yǎng)中應(yīng)添加符合推薦的多種維生素的觀念仍需加強(qiáng)。
[Abstract]:Objective: to understand the use of vitamins in the prescription of parenteral nutrition in China. Methods: "parenteral nutrition" or "intravenous nutrition" and "prescription analysis" or "rational medication" or "doctor's order analysis" were used as the key words for the rational use of vitamins in parenteral nutrition in China. To search the literature on the rationality of parenteral nutrition prescription reported from 2006 to 2015 in China Zhiwang and Wanfang database. The use of vitamins in the literature was analyzed and summarized. Results: a total of 34 articles were included, involving a total of 158,874 prescriptions. Of these, 145,400 were prescribed with vitamins; There were 73,508 prescriptions using 12 or 13 vitamins, 53,388 or 33.60) prescriptions were considered overdose by the authors of the literature. (18,504) prescription vitamin supplements are incomplete. No vitamins were added to the prescription. The proportion of 12 or 13 vitamins added in the northern part of our country was the highest (52.00). ... the "three A" hospitals added 12 or 13 vitamins. The authors believe that excessive use of vitamins and water-soluble vitamins. The proportion of fat-soluble vitamins that were not completely added was lower than that of the hospitals below "three A". The prescription department with 12 or 13 essential vitamins from high to low was oncology department (97.72%). Conclusion: the proportion of the standard use of vitamins in the prescriptions of parenteral nutrition in China is on the low side. The idea that parenteral nutrition should be supplemented with recommended multivitamins still needs to be strengthened.
