【摘要】：目的通過(guò)臨床藥師干預(yù),促進(jìn)我院輔助用藥的規(guī)范性、安全性和經(jīng)濟(jì)性,達(dá)到合理用藥的目的。方法選取我院2015年2月~2016年1月輔助用藥醫(yī)囑,對(duì)其進(jìn)行專(zhuān)項(xiàng)點(diǎn)評(píng),將其中發(fā)現(xiàn)的典型問(wèn)題通過(guò)臨床藥師查房宣教的方式加以干預(yù),評(píng)價(jià)該類(lèi)藥在臨床應(yīng)用中的潛在風(fēng)險(xiǎn)和經(jīng)干預(yù)后的效果。結(jié)果我院輔助用藥中應(yīng)用范圍最廣的是中藥注射劑及其衍生物,審核醫(yī)囑問(wèn)題出現(xiàn)較多的分別為丹參川芎嗪注射液、轉(zhuǎn)化糖電解質(zhì)、脂溶(Ⅱ)/水溶性維生素,問(wèn)題醫(yī)囑比例分別為90.0%、84.6%和83.8%。存在的主要問(wèn)題是超說(shuō)明書(shū)用法用量、適應(yīng)證不適宜、聯(lián)合用藥不適宜、溶媒選擇不適宜等。結(jié)論通過(guò)臨床藥師干預(yù)后,我院輔助用藥情況得到明顯改善,用藥更加合理化,從而能夠更好地服務(wù)于患者。
