【摘要】：目的通過臨床藥師干預,促進我院輔助用藥的規(guī)范性、安全性和經(jīng)濟性,達到合理用藥的目的。方法選取我院2015年2月~2016年1月輔助用藥醫(yī)囑,對其進行專項點評,將其中發(fā)現(xiàn)的典型問題通過臨床藥師查房宣教的方式加以干預,評價該類藥在臨床應用中的潛在風險和經(jīng)干預后的效果。結果我院輔助用藥中應用范圍最廣的是中藥注射劑及其衍生物,審核醫(yī)囑問題出現(xiàn)較多的分別為丹參川芎嗪注射液、轉(zhuǎn)化糖電解質(zhì)、脂溶(Ⅱ)/水溶性維生素,問題醫(yī)囑比例分別為90.0%、84.6%和83.8%。存在的主要問題是超說明書用法用量、適應證不適宜、聯(lián)合用藥不適宜、溶媒選擇不適宜等。結論通過臨床藥師干預后,我院輔助用藥情況得到明顯改善,用藥更加合理化,從而能夠更好地服務于患者。
