【摘要】：目的:促進(jìn)化療藥物醫(yī)囑開具的規(guī)范化、合理化,提高化療藥物臨床使用的安全性。方法:通過開展相關(guān)現(xiàn)狀調(diào)查工作,確定在醫(yī)院信息系統(tǒng)(HIS)中設(shè)計(jì)化療藥物醫(yī)囑管理模塊時(shí)應(yīng)涉及和考慮的因素和信息;規(guī)范并確定醫(yī)囑開具時(shí)參考的標(biāo)準(zhǔn)化療方案,我院維護(hù)基礎(chǔ)數(shù)據(jù)、設(shè)定菜單,基于HIS拓展設(shè)計(jì)患者信息自動(dòng)導(dǎo)入、藥物及治療方案選擇、警示等功能,實(shí)現(xiàn)化療藥物醫(yī)囑管理模塊在HIS中的對(duì)接和有效應(yīng)用;通過統(tǒng)計(jì)模塊運(yùn)行后2年內(nèi)每月化療藥物醫(yī)囑不合理率,評(píng)價(jià)其應(yīng)用效果。結(jié)果與結(jié)論:我院開發(fā)的化療藥物醫(yī)囑管理模塊能夠?qū)崿F(xiàn)醫(yī)師授權(quán)管理、自動(dòng)導(dǎo)入患者相關(guān)信息、預(yù)置化療方案以方便醫(yī)師錄入與選擇等功能;該模塊運(yùn)行后,不合理化療藥物醫(yī)囑從初期(2014年6月)的28條(1.15%)下降至2年后(2016年6月)的4條(0.14%)。規(guī)范了化療藥物醫(yī)囑開具過程,并初步實(shí)現(xiàn)了信息化管理,提高了醫(yī)囑錄入效率及準(zhǔn)確性,為規(guī)范醫(yī)師診療行為和規(guī)避醫(yī)院風(fēng)險(xiǎn)提供了保障和支持,促進(jìn)了化療藥物的正確、安全、合理使用。
[Abstract]:Objective: to promote the standardization and rationalization of prescriptions of chemotherapeutic drugs and to improve the safety of clinical use of chemotherapeutic drugs. To determine the factors and information that should be taken into account when designing the module of chemotherapeutic doctor's order management in hospital information system (HIS); Standardize and determine the standard chemotherapy plan, maintain the basic data, set the menu, expand the design of patient information based on HIS, drug and treatment program selection, warning and other functions. To realize the docking and effective application of chemotherapeutic medicine management module in HIS; The unreasonable rate of chemotherapeutic orders per month within 2 years after the operation of the statistical module was used to evaluate the application effect. Results and conclusion: the management module of chemotherapeutic orders developed in our hospital can realize the management of doctors' authorization. The patient's relevant information was automatically imported and the chemotherapy protocol was preset to facilitate the doctor's input and selection. After the module runs. Unreasonable chemotherapeutic orders decreased from 28 in the initial period (June 2014) to 4 in 2 years (June 2016). The procedure of prescribing chemotherapeutic orders was standardized. It has realized the information management, improved the efficiency and accuracy of the doctor's order input, provided the guarantee and support for standardizing the doctor's diagnosis and treatment behavior and evading the hospital risk, promoted the correct, safe and reasonable use of the chemotherapeutic drugs.
