滲出性年齡相關(guān)性黃斑變性在眼底自發(fā)熒光及紅外光中的影像特征

【摘要】:目的:觀察和比較滲出性年齡相關(guān)性黃斑變性(age-related maculardegeneration,AMD)不同類型的眼底自發(fā)熒光(autofluorescence,AF)和紅外光(infrared ray,IR)的影像特征。 方法:對(duì)23眼滲出性AMD患者進(jìn)行自發(fā)熒光(AF)、紅外光(IR)、熒光素眼底血管造影(FFA)、吲哚菁綠脈絡(luò)膜血管造影(ICGA)、光學(xué)相干斷層掃描(OCT)檢查,觀察、比較不同類型滲出性AMD患者眼底AF及IR特點(diǎn)。 結(jié)果:患者23眼中,有7眼為典型性CNV,9眼為隱匿性CNV,7眼為混合性CNV。在典型性CNV中,AF特征:7眼(100%)表現(xiàn)為病灶區(qū)明顯低熒光,5眼(71.42%)見病灶外緣環(huán)繞相對(duì)增高的熒光環(huán)。IR特征:病灶區(qū)內(nèi)主要為低熒光,在其外圍圍繞著環(huán)狀或半環(huán)狀高熒光。在隱匿型CNV中,,AF特征:隱匿性CNV有8例(88.89%)表現(xiàn)為斑駁樣點(diǎn)狀或片狀低熒光,1例熒光無顯著變化。IR特征:主要表現(xiàn)為散在的點(diǎn)狀不均勻的高熒光灶,邊界模糊不清。在混合型CNV中,AF特征:7例(100%)表現(xiàn)為片狀低熒光區(qū)伴多發(fā)散在的斑駁樣低熒光。IR特征:FFA滲漏區(qū)表現(xiàn)為低熒光,周圍伴有散在高熒光。 結(jié)論:眼底自發(fā)熒光和紅外光是目前集多種優(yōu)點(diǎn)于一體的新型眼底成像技術(shù)。滲出性AMD患者眼底表現(xiàn)復(fù)雜多變,每種類型具有其不同的特征,因此AF及IR形態(tài)也多種多樣。動(dòng)態(tài)監(jiān)測這些異常的AF及IR影像特點(diǎn)對(duì)評(píng)價(jià)不同類型滲出性年齡相關(guān)性黃斑變性的發(fā)病進(jìn)展過程具有一定的臨床意義。
[Abstract]:Aim: to observe and compare the imaging features of different types of fundus autofluorescence (autofluorescence,AF) and infrared light (infrared ray,IR) in exudative age related macular degeneration (age-related maculardegeneration,AMD). Methods: 23 eyes with exudative AMD were examined by (ICGA), optical coherence tomography (OCT) with (AF), infrared (IR), fluorescein angiography and (FFA), indocyanine green choroidal angiography. To compare the characteristics of fundus AF and IR in patients with different types of exudative AMD. Results: in 23 eyes, 7 eyes were typical CNV,9, occult CNV,7 eyes were mixed CNV.. In typical CNV, AF showed obvious hypofluorescence in 7 eyes (100%), low fluorescence in 5 eyes (71.42%) and low fluorescence in 5 eyes (71.42%). It is surrounded by annular or semi-annular high fluorescence. In occult CNV, AF features: 8 cases (88.89%) of occult CNV showed mottled dot or flake low fluorescence, while 1 case showed no significant change in fluorescence. The boundary is blurred. In mixed CNV, AF was found in 7 cases (100%) with flake low fluorescence region and multiple diffuse mottled low fluorescence, while IR showed low fluorescence in FFA leakage region and high fluorescence around it. Conclusion: fundus autofluorescence and infrared light are new fundus imaging techniques. The fundus manifestations of exudative AMD patients are complex and variable, and each type has its own characteristics. Therefore, the morphology of AF and IR is also varied. Dynamic monitoring of these abnormal AF and IR imaging features has certain clinical significance in evaluating the pathogenesis and progression of different types of exudative age-related macular degeneration.
