【摘要】：目的初步探討射頻消融犬心室流出道間隔對(duì)左心室結(jié)構(gòu)與功能的影響。方法以健康實(shí)驗(yàn)犬為模型,射頻消融心室流出道間隔處肌肉。術(shù)前經(jīng)胸超聲檢測(cè)左心室流出道寬度、收縮期室間隔厚度和射血分?jǐn)?shù)等指標(biāo),消融成功后即刻檢測(cè)超聲指標(biāo),術(shù)后1、3個(gè)月分別復(fù)查存活犬超聲指標(biāo),作組織病理學(xué)檢查。結(jié)果 10只實(shí)驗(yàn)犬中9只消融成功,1只術(shù)中發(fā)生室顫死亡;1只消融成功但麻醉過深,出現(xiàn)呼吸抑制死亡。手術(shù)即刻成功率為80%,8只術(shù)后存活至預(yù)定隨訪時(shí)間點(diǎn)。與術(shù)前相比,存活犬術(shù)后即刻、術(shù)后1個(gè)月、術(shù)后3個(gè)月左心室流出道寬度、收縮期室間隔厚度差異均有統(tǒng)計(jì)學(xué)意義(P0.05),射血分?jǐn)?shù)差異均無統(tǒng)計(jì)學(xué)意義(P0.05);組織病理學(xué)檢查可見明顯改變;心電圖、血壓未見明顯變化。結(jié)論初步驗(yàn)證射頻消融實(shí)驗(yàn)犬心室流出道間隔心肌具有一定的安全性和有效性,為進(jìn)一步動(dòng)物實(shí)驗(yàn)和臨床試驗(yàn)提供了有用參數(shù)和實(shí)驗(yàn)依據(jù)。
[Abstract]:Objective to investigate the effect of radiofrequency ablation of ventricular outflow tract septum on left ventricular structure and function in dogs. Methods the muscle of ventricular outflow tract septum was ablation by radiofrequency ablation in healthy dogs. The width of left ventricular outflow tract, the thickness of interventricular septum and ejection fraction were measured by transthoracic ultrasound before operation, and the ultrasonic indexes were measured immediately after successful ablation. The ultrasonic indexes of surviving dogs were reviewed 1 month and 3 months after operation, and the pathological examination was performed. Results among 10 experimental dogs, 9 dogs underwent successful ablation, 1 died of ventricular fibrillation during operation, and 1 dog died of respiratory inhibition due to successful ablation but deep anesthesia. The immediate success rate was 80%. Eight patients survived until the scheduled follow-up time. Compared with before operation, there were significant differences in left ventricular outflow tract width and systolic interventricular septum thickness between survival dogs immediately after operation, 1 month after operation and 3 months after operation (P 0.05), but there was no significant difference in ejection fraction (P 0.05). There were significant changes in histopathological examination and no significant changes in electrocardiogram and blood pressure. Conclusion radiofrequency ablation is safe and effective in canine ventricular outflow septum myocardium, which provides useful parameters and experimental basis for further animal and clinical trials.
