【摘要】：射血分數(shù)(ejection fraction,EF)是每搏輸出量與心室舒張末容積的百分比,由于沒有時間限制,不能反映心臟射血的敏捷性與時效性,為泵血功能的總體反映。在目前射血分數(shù)保留的心衰(heart failure with preserved EF,HFp EF)患者日益增多的條件下,EF臨床意義有限。而泵血是心臟最基本的功能,在EF無法評估心功能的條件下,如何用其他相對客觀且簡單指標進行心功能評估于臨床顯得尤為重要。雖已有較多相關研究,但結果均不理想。而心電圖QRS波時限與心肌收縮時間有較好的相關性,滿足時間射血分數(shù)(timed EF,TEF)中時間因素的要求,符合用TEF代替?zhèn)鹘y(tǒng)的EF的設想,即以EF與心電圖QRS波時限比值評估心功能。它既包含心臟原有的EF,又加入特定的時間限制,能較全面而客觀地反映心臟實際泵血能力,特別是對HFp EF患者和心衰早期患者心功能的評估,是心臟泵血功能評估新指標的有益嘗試和探討。
[Abstract]:Ejection fraction (ejection fraction,EF) is the percentage of stroke volume and ventricular end-diastolic volume. Because there is no time limit, it can not reflect the agility and timeliness of cardiac ejection, and it is the overall reflection of pumping function. Under the increasing number of patients with heart failure (heart failure with preserved EF,HFp EF with ejection fraction retention, the clinical significance of EF is limited. Blood pumping is the most basic function of the heart. Under the condition that EF can not evaluate the cardiac function, how to evaluate the cardiac function with other relatively objective and simple indexes is particularly important in clinic. Although there have been many related studies, the results are not ideal. However, there is a good correlation between the duration of QRS wave and the time of myocardial contraction, which meets the requirements of time factor in time ejection fraction (timed EF,TEF) and accords with the assumption of using TEF instead of traditional EF, that is, the ratio of EF to QRS wave duration of electrocardiogram is used to evaluate cardiac function. It not only contains the original EF, of the heart, but also adds a specific time limit, which can comprehensively and objectively reflect the actual pumping ability of the heart, especially the evaluation of cardiac function in patients with HFp EF and early patients with heart failure. It is a useful attempt and discussion for the evaluation of new indexes of cardiac pumping function.
