【摘要】：微量白蛋白尿(microalbuminuria,MAU)的出現(xiàn)不僅提示腎臟血管內(nèi)皮受損,還可作為全身血管內(nèi)皮受損的信號(hào),是血管內(nèi)皮功能障礙及血管炎癥反應(yīng)的早期預(yù)測(cè)因子。微量白蛋白尿在心血管疾病中,與心血管危險(xiǎn)因素及心血管疾病關(guān)系密切,對(duì)動(dòng)脈粥樣硬化、冠心病、高血壓、心力衰竭的病情評(píng)估及預(yù)后有重要的價(jià)值。但其是如何與心血管疾病有關(guān)的,目前尚未闡明,仍需要深入的基礎(chǔ)研究。對(duì)于如何減少蛋白尿方面,目前在實(shí)際醫(yī)療工作中應(yīng)用藥物主要有四種:血管緊張素轉(zhuǎn)換酶抑制劑、血管緊張素II受體拮抗劑、腎素抑制劑、醛固酮受體拮抗劑。但是否可通過減低蛋白尿來明顯延緩心血管疾病的進(jìn)展和改善患者預(yù)后,為心血管疾病的預(yù)防與治療帶來新的思路,仍需要更多的大數(shù)據(jù)樣本及高質(zhì)量的臨床試驗(yàn)來證實(shí)。
[Abstract]:The appearance of microalbuminuria (microalbuminuria,MAU) not only suggests that renal vascular endothelial damage, but also can be used as a signal of systemic vascular endothelial damage, and is an early predictor of vascular endothelial dysfunction and vascular inflammation. Microalbuminuria is closely related to cardiovascular risk factors and cardiovascular diseases in cardiovascular diseases. It is of great value in the evaluation and prognosis of atherosclerosis, coronary heart disease, hypertension and heart failure. However, how it is related to cardiovascular disease has not yet been clarified, and further basic research is still needed. At present, there are four kinds of drugs used in practical medical work: angiotensin converting enzyme inhibitor, angiotensin II receptor antagonist, renin inhibitor and aldosterone receptor antagonist. However, whether we can significantly delay the progress of cardiovascular disease and improve the prognosis of patients by reducing albuminuria, and bring new ideas for the prevention and treatment of cardiovascular disease, more big data samples and high quality clinical trials are still needed to confirm it.
