Allo-HSCT治療重再的臨床研究及臍帶間充質(zhì)干細(xì)胞和他克莫司在抑制GVHD中相互作用的研究

【摘要】:目的探討改良預(yù)處理方案用于HLA同胞全相合(MSD)、親緣單倍體相合(HFD)造血干細(xì)胞移植(HSCT)治療重型再生障礙性貧血(SAA)的安全性和可行性。方法回顧性分析2011年1月至2016年6月解放軍總醫(yī)院第一附屬醫(yī)院行Allo-HSCT治療的56例SAA患者臨床資料(HFD 38例/MSD 18例),比較兩種移植方式對(duì)造血重建、移植物抗宿主病(GVHD)、移植相關(guān)毒性(TRT)及總生存(OS)的影響。改良FC /ATG預(yù)處理方案包括減低環(huán)磷酰胺(Cy)劑量(總量100 mg /kg)和給予第三方間充質(zhì)干細(xì)胞輸注。結(jié)果56例行MSD-HSCT或HFD-HSCT的患者全部造血重建。比較中性粒細(xì)胞植入時(shí)間(P=0.58)、血小板植入時(shí)間(P=0.61)、Ⅲ-Ⅳ度aGVHD發(fā)生率(P=0.83)、廣泛性cGVHD發(fā)生率(P=0.95),重度TRT發(fā)生率(P=0.69),均顯示差別無統(tǒng)計(jì)學(xué)意義。隨訪32(2-66)個(gè)月,48例存活至今,HFD-HSCT組和MSD-HSCT組1年OS分別為86%和89%,差異無統(tǒng)計(jì)學(xué)意義(P=0.58)。結(jié)論采用改良預(yù)處理方案的Allo-HSCT治療SAA植入穩(wěn)定、毒副作用弱,GVHD發(fā)生率低,預(yù)后良好;同時(shí)對(duì)于無MSD的SAA患者,HFD-HSCT療效與MSD-HSCT相當(dāng),可以作為替代治療。目的本實(shí)驗(yàn)擬觀察他克莫司(FK-506)對(duì)臍帶間充質(zhì)干細(xì)胞(UC-MSC)活化增殖的影響,研究FK-506與UC-MSC在免疫抑制方面作用機(jī)制及是否有協(xié)同作用。為臨床預(yù)防和治療GVHD提供一定理論基礎(chǔ)。方法流式細(xì)胞儀檢測(cè)UC-MSC的表面抗原;CCK8方法檢測(cè)FK-506對(duì)UC-MSC增殖的影響;流式細(xì)胞儀檢測(cè)FK-506對(duì)UC-MSC凋亡及細(xì)胞周期的影響;UC-MSC或/和不同濃度FK-506與PHA刺激的外周血單個(gè)核細(xì)胞(PBSC)共培養(yǎng),用ELIS A方法檢測(cè)培養(yǎng)上清中IFN-γ的分泌情況。結(jié)果1. UC-MSC表面表達(dá)CD44、CD73、CD90、CD105、CD29,不表達(dá)CD11b、CD19、CD34、CD45、CD31、HLA-DR。2.不同濃度FK-506與UC-MSC共培養(yǎng)24h和48h,隨著FK-506濃度增加,UC-MSC增殖率降低,但各實(shí)驗(yàn)組與無藥對(duì)照組之間差異無統(tǒng)計(jì)學(xué)意義。3.不同濃度FK-506與UC-MSC共培養(yǎng)48h,流式細(xì)胞儀檢測(cè)UC-MSC凋亡,各實(shí)驗(yàn)組與無藥對(duì)照組之間差異無統(tǒng)計(jì)學(xué)意義。4.不同濃度FK-506與UC-MSC共培養(yǎng)48h,隨著FK-506濃度增加,UC-MSC細(xì)胞周期GO/G1進(jìn)入S期比例增加,各實(shí)驗(yàn)組與無藥對(duì)照組間差異有統(tǒng)計(jì)學(xué)意義。5.不同濃度FK-506與PHA刺激下PBSC共培養(yǎng)72h,隨著FK-506濃度增加,上清中IFN-γ減少,即IFN-γ分泌與FK-506濃度呈負(fù)相關(guān),且無藥對(duì)照組與實(shí)驗(yàn)組之間差異有統(tǒng)計(jì)學(xué)意義。6.UC-MSC與PHA刺激下PBSC不同濃度比共培養(yǎng)72h,隨著UC-MSC濃度增加,上清中IFN-γ減少,即IFN-γ分泌與UC-MSC濃度呈負(fù)相關(guān),且各實(shí)驗(yàn)組與無藥對(duì)照組之間差異有統(tǒng)計(jì)學(xué)意義。7.PHA刺激的PBSC中,FK-506組、UC-MSC組、FK-506+UC-MSC組共培養(yǎng)72h,上清中IFN-γ分泌情況為:PBSC組FK-506+PBSC組UC-MSC+PBSC組FK-506+UC-MSC組+ PBSC組,且各實(shí)驗(yàn)組與對(duì)照組間差異有統(tǒng)計(jì)學(xué)意義。結(jié)論FK-506對(duì)UC-MSC增殖及凋亡無抑制作用,但能促進(jìn)其細(xì)胞周期GO/G1進(jìn)入S期。UC-MSC與FK-506均能抑制PHA刺激下PBSC的IFN-γ的分泌,提示兩者對(duì)T淋巴細(xì)胞的活化增殖有免疫抑制作用,且兩者在免疫抑制方面具有協(xié)同作用。為臨床預(yù)防和治療GVHD提供一定理論基礎(chǔ)
