【摘要】：目的:比較單純抗凝與深靜脈置管溶栓治療急性上肢深靜脈血栓形成的近期療效,評(píng)價(jià)深靜脈置管溶栓的應(yīng)用價(jià)值。方法:回顧性分析2009年7月至2015年10月我院收治的33例急性上肢深靜脈血栓患者,分為單純抗凝組(A組,22例)和深靜脈置管溶栓組(B組,11例),對(duì)比兩組視覺模擬評(píng)分、上肢周徑治療前后平均減小值及主觀癥狀評(píng)分的差異。結(jié)果:兩組的上肢周徑治療前后平均減小值,B組為(1.2±0.3)cm,好于A組(0.5±0.1)cm,差異有統(tǒng)計(jì)學(xué)意義(P0.05),主觀癥狀評(píng)分A組3.5±1.2;B組1.1±1.2。差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:初步研究結(jié)果提示急性上肢深靜脈血栓導(dǎo)管直接溶栓治療較單純抗凝治療,患肢腫痛癥狀改善更明顯,是一種安全有效的方法。
[Abstract]:Aim: to compare the short-term efficacy of anticoagulation and deep venous thrombolysis in the treatment of acute upper limb deep venous thrombosis (DVT) and to evaluate the clinical value of thrombolytic therapy. Methods: from July 2009 to October 2015, 33 patients with acute deep venous thrombosis of upper extremities were divided into two groups: group A (n = 22) and group B (n = 11). Visual analogue scores, average reduction of upper limb circumference and subjective symptom scores before and after treatment were compared between the two groups. Results: the average decrease of upper limb circumference in group B was significantly better than that in group A (1.2 鹵0.3 cm, vs (0.5 鹵0.1) cm, P0.05). The subjective symptom score of group A was 3.5 鹵1.2; Group B 1.1 鹵1.2. The difference was statistically significant (P0.05). Conclusion: the primary results suggest that direct thrombolytic therapy of acute upper extremity deep venous thrombosis is more effective and safe than anticoagulation therapy alone, and the symptoms of swelling and pain in the affected limbs are improved more obviously.
