【摘要】：目的比較血漿致動(dòng)脈硬化指數(shù)(AIP)與脈搏波傳導(dǎo)速度(PWV)對(duì)冠心病預(yù)測(cè)的準(zhǔn)確性,探討AIP檢測(cè)用于預(yù)測(cè)冠心病的可行性。方法收集在我院住院且疑診為冠心病并行冠狀動(dòng)脈造影(CAG)檢查的患者316例,其中男196例,女120例,年齡為60.60±9.83歲,根據(jù)CAG結(jié)果分為冠心病組(n=218)和非冠心病組(n=98),采用Logistic回歸方法篩選冠心病的獨(dú)立危險(xiǎn)因素,并分析AIP與PWV對(duì)冠心病診斷的預(yù)測(cè)價(jià)值。結(jié)果冠心病組患者AIP和PWV均明顯高于無(wú)冠心病組(P0.01);Logistic多因素回歸分析顯示,AIP和PWV是冠心病的獨(dú)立危險(xiǎn)因素(P0.01),且二者呈正相關(guān)(r=0.830,P0.01);在AIP和PWV預(yù)測(cè)冠心病能力的受試者工作特征曲線(ROC)中,其曲線下面積分別為0.764(95%CI為0.683~0.802,P0.01)及0.721(95%CI為0.642~0.784,P0.01);當(dāng)AIP取0.161及PWV取1362 cm/s最佳臨界值時(shí),二者對(duì)冠心病診斷的敏感性、特異性、準(zhǔn)確性均無(wú)差別(P0.05)。結(jié)論 AIP可用于冠心病的預(yù)測(cè),尤其是當(dāng)AIP水平超過(guò)0.161時(shí)診斷冠心病的可能性大。
[Abstract]:Objective to compare the accuracy of plasma arteriosclerosis index (AIP) and pulse wave conduction velocity (PWV) in predicting coronary heart disease (CHD). Methods 316 patients (196 males and 120 females, aged 60.60 鹵9.83 years) with suspected coronary artery disease (CHD) underwent coronary angiography (CAG) in our hospital. According to the results of CAG, the patients were divided into two groups: coronary heart disease group (nhm218) and non-coronary heart disease group (NCN98). The independent risk factors of coronary heart disease were screened by Logistic regression method, and the predictive value of AIP and PWV in the diagnosis of coronary heart disease was analyzed. Results AIP and PWV were significantly higher in patients with coronary heart disease than those without coronary heart disease (P0.01); Logistic multivariate regression analysis showed that AIP and PWV were independent risk factors of coronary heart disease (P0.01), and there was a positive correlation between AIP and PWV (r = 0.830, P 0.01). In the operating characteristic curve (ROC) of AIP and PWV, the area under the curve was 0.764 (95%CI = 0.683) and 0.721 (95%CI = 0.642n, 0.784g, P0.01). When AIP was 0.161 and PWV was 1362 cm/s, there was no difference in sensitivity, specificity and accuracy in the diagnosis of coronary heart disease (P0.05). Conclusion AIP can be used to predict coronary heart disease, especially when the level of AIP is higher than 0.161.
