血清γ谷氨酰轉移酶水平與急性心力衰竭患者預后的相關性研究

【摘要】:目的:評價血清γ谷氨酰轉移酶(Serum gamma glutamine transferase,GGT)水平與急性心力衰竭(acute heart failure, AHF)患者預后的相關性。方法:選取2012年9月至2013年1月期間于我院心臟中心住院的急性左心室收縮功能衰竭患者188例(男性78例,女性110例)臨床資料進行回顧性分析,根據(jù)患者預后死亡或存活分為死亡組及存活組,其中死亡組36例,男14例,女22例,平均年齡(67.28±11.04)歲,存活組152例,男64例,女88例,平均年齡(63.95±12.42)歲,分析兩組患者的基本臨床資料及院內(nèi)死亡率。結果:兩組間射血分數(shù)、心房纖顫、民族、高血壓、高密度脂蛋白、低密度脂蛋白、血尿素氮、血肌酐、肌酸激酶、肌酸激酶同工酶、總膽汁酸、心房腦鈉肽均有顯著差異。死亡組與存活組比較,兩組患者的血清γ谷氨酰轉移酶分別為39.24±5.90和31.03±10.22,有顯著差異;兩組間性別、年齡、空腹血糖、脂蛋白(a)、總蛋白、甘油三酯、總膽固醇、尿酸、糖尿病、心力衰竭分級、藥物應用均無明顯差異;根據(jù)血清γ谷氨酰轉移酶水平患者也分為兩組,高血清γ谷氨酰轉移酶組與低血清γ谷氨酰轉移酶組死亡率比例為(15.3%vs4.6%)。Logistic回歸分析顯示血清γ谷氨酰轉移酶為急性心力衰竭的危險因素P0.05。用ROC曲線對評價血清γ谷氨酰轉移酶對于急性心力衰竭死亡死亡的診斷價值,敏感度78.9%,特異度66.7%,cutoff值35.5 U/L,曲線下的面積為0.759,95%的置信區(qū)間0.662-0.859,P0.05。結論:血清γ谷氨酰轉移酶水平與急性心力衰竭患者預后有相關性,具有一定的臨床應用價值。
[Abstract]:Objective: to evaluate the correlation between serum 緯-glutamyltransferase (Serum gamma glutamine transferase,GGT) level and prognosis of patients with acute heart failure (acute heart failure, AHF). Methods: the clinical data of 188 patients with acute left ventricular systolic failure (78 males and 110 females) hospitalized in our heart center from September 2012 to January 2013 were retrospectively analyzed. According to the prognosis, the patients were divided into death group (n = 36) and survival group (n = 36, male 14, female = 22, mean age: (67.28 鹵11.04) years), survival group (n = 152), male (64), female (88), mean age (63.95 鹵12.42). The basic clinical data and hospital mortality of the two groups were analyzed. Results: there were significant differences in ejection fraction, atrial fibrillation, nationality, hypertension, high density lipoprotein, low density lipoprotein, blood urea nitrogen, serum creatinine, creatine kinase isoenzyme, total bile acid and atrial brain natriuretic peptide between the two groups. The serum levels of 緯 -glutamyltransferase were 39.24 鹵5.90 and 31.03 鹵10.22 in the death group and the survival group, respectively. There were no significant differences in sex, age, fasting blood glucose, lipoprotein (a), total protein, triglyceride, total cholesterol, uric acid, diabetes mellitus, heart failure grade and drug use between the two groups. According to the level of serum 緯 -glutamyl transferase, patients were also divided into two groups. The mortality ratio of high serum 緯 -glutamyltransferase group and low serum 緯 -glutamyltransferase group was 15.3vs4.6%). Logistic regression analysis showed that serum 緯 -glutamyltransferase was a risk factor for acute heart failure (P0.05). The ROC curve was used to evaluate the diagnostic value of serum 緯 -glutamyltransferase in the death of acute heart failure. The sensitivity was 78.9 and the specificity was 66.7U / L, which was 35.5 UL / L, respectively. The area under the curve is 0.759V 95% confidence interval 0.662-0.859g P0.05. Conclusion: the level of serum 緯-glutamyl transferase is correlated with the prognosis of patients with acute heart failure.
