TCRP術(shù)后LNG-IUS放置對子宮內(nèi)膜息肉合并中度貧血患者內(nèi)膜厚度、復(fù)發(fā)率及并發(fā)癥的影響

【摘要】：目的探討宮腔鏡息肉電切術(shù)(TCRP)后左炔諾孕酮宮內(nèi)緩釋系統(tǒng)(LNG-IUS)放置對子宮內(nèi)膜息肉合并中度貧血患者內(nèi)膜厚度、復(fù)發(fā)率及并發(fā)癥的影響。方法研究對象選取該院2014年1月-2016年1月收治子宮內(nèi)膜息肉合并中度貧血患者共140例,以隨機數(shù)字表法分為對照組(70例)和觀察組(70例),分別給予TCRP術(shù)單用和在此基礎(chǔ)上術(shù)后加用LNG-IUS放置;比較兩組患者治療前后子宮內(nèi)膜厚度、血紅蛋白水平、畫報失血評估表(PBAC)評分、復(fù)發(fā)率及并發(fā)癥發(fā)生率等。結(jié)果觀察組患者治療后子宮內(nèi)膜厚度明顯少于對照組和治療前(P0.05);觀察組患者治療后血紅蛋白水平和PBAC評分均明顯優(yōu)于對照組和治療前(P0.05);觀察組患者隨訪復(fù)發(fā)率明顯低于對照組(P0.05);同時兩組患者并發(fā)癥發(fā)生率比較差異無統(tǒng)計學(xué)意義(P0.05)。結(jié)論 TCRP術(shù)后LNG-IUS放置治療子宮內(nèi)膜息肉合并中度貧血可有效降低子宮內(nèi)膜厚度,減少子宮出血量,避免遠期復(fù)發(fā),且未增加相關(guān)并發(fā)癥發(fā)生概率。
[Abstract]:Objective to investigate the effect of intrauterine sustained release system (LNG-IUS) placement of levonorgestrel (LNG-IUS) on endometrial thickness, recurrence rate and complications in patients with moderate anemia after hysteroscopic polyp resection (TCRP). Methods from January 2014 to January 2016, 140 patients with endometrial polyps with moderate anemia were randomly divided into control group (n = 70) and observation group (n = 70). TCRP was given alone and LNG-IUS was added after operation. The endometrial thickness, hemoglobin level, (PBAC) score, recurrence rate and complication rate were compared between the two groups before and after treatment. Results the thickness of endometrium in the observation group was significantly lower than that in the control group and before treatment (P0.05), and the hemoglobin level and PBAC score in the observation group were significantly better than those in the control group and before treatment (P0.05). The recurrence rate of the patients in the observation group was significantly lower than that in the control group (P0.05), and there was no significant difference in the incidence of complications between the two groups (P0.05). Conclusion LNG-IUS placement after TCRP in the treatment of endometrial polyps with moderate anemia can effectively reduce endometrial thickness, reduce the amount of uterine bleeding, avoid long-term recurrence, and do not increase the probability of complications.
