接觸壓力導(dǎo)管在零射線下消融陣發(fā)性房室結(jié)折返性心動(dòng)過速中的應(yīng)用

【摘要】:目的:本研究通過比較在零射線下使用壓力導(dǎo)管(Smart Touch,Biosense Webster)與常規(guī)X線指導(dǎo)下使用普通消融導(dǎo)管消融陣發(fā)性房室結(jié)折返性心動(dòng)過速(atrioventricular nodal reentrant tachycardial,AVNRT)來評(píng)價(jià)其安全性、可行性及有效性。實(shí)驗(yàn)方法:分析2015年10月至2016年9月在南方醫(yī)院因AVNRT行零射線射頻消融治療的患者28例,即運(yùn)用Carto3系統(tǒng)重建右心房三維模型后使用壓力導(dǎo)管在零射線下行導(dǎo)管消融(實(shí)驗(yàn)組)。對(duì)照組是既往在南方醫(yī)院因AVNRT行常規(guī)X線指導(dǎo)下導(dǎo)管消融治療的患者30例。比較兩組的消融功率、放電次數(shù)、手術(shù)即時(shí)成功率、手術(shù)總時(shí)間、X線曝光時(shí)間、并發(fā)癥等。結(jié)果:兩組的手術(shù)成功率均為100%;實(shí)驗(yàn)組與對(duì)照在手術(shù)總時(shí)間(實(shí)驗(yàn)組86.7±11.9 min VS對(duì)照組93.2±11.3min,P=0.37)、并發(fā)癥上差別無統(tǒng)計(jì)學(xué)意義(P=0.37)。與對(duì)照組相比,實(shí)驗(yàn)組的消融功率(實(shí)驗(yàn)組25.7±6.OW VS對(duì)照組35.7±10.0W,P0.01)明顯較低,放電次數(shù)(實(shí)驗(yàn)組4.8±1.1次VS對(duì)照組7.9±3.2次,P0.01)也明顯較少,實(shí)驗(yàn)組全程零射線。結(jié)論:壓力導(dǎo)管零射線消融AVNRT是安全有效可行的,全程零射線能避免射線對(duì)醫(yī)患雙方帶來的危害。
[Abstract]:Objective: to evaluate the safety of paroxysmal atrioventricular nodal reentrant tachycardia (atrioventricular nodal reentrant tachycardial,AVNRT) by comparing the ablation of paroxysmal atrioventricular nodal reentrant tachycardia (atrioventricular nodal reentrant tachycardial,AVNRT) with the use of pressure catheter (Smart Touch,Biosense Webster) under the guidance of conventional radiography. Feasibility and effectiveness. Methods: Twenty-eight patients underwent zero-ray radiofrequency ablation (AVNRT) in Southern Hospital from October 2015 to September 2016 were analyzed. The three dimensional model of right atrium was reconstructed by Carto3 system and then catheter ablation was performed with pressure catheter (experimental group). The control group consisted of 30 patients who had been treated with conventional X-ray ablation under the guidance of conventional X-ray therapy due to AVNRT in Southern Hospital. The ablation power, discharge frequency, immediate success rate, total operation time, X-ray exposure time, complications were compared between the two groups. Results: the successful rate of operation was 100% in both groups, and the total operation time was 86.7 鹵11.9 min VS in the experimental group and 93.2 鹵11.3min in the control group. There was no significant difference in complications between the two groups (P0. 37).
