成年人蔬菜水果攝入與高血壓關(guān)系的研究

【摘要】:目的:蔬菜水果攝入量與高血壓發(fā)病風(fēng)險之間的關(guān)系尚不明確。本文通過一個現(xiàn)況調(diào)查和一個Meta分析來探討蔬菜和(或)水果以及分類蔬菜水果攝入量與高血壓之間的關(guān)系。方法:在現(xiàn)況調(diào)查中,關(guān)于總的蔬菜水果攝入量與高血壓之間關(guān)系的研究,數(shù)據(jù)來自美國國民健康和營養(yǎng)體格檢查2007-2010年12002個成年人個體,采用單因素和多因素logistic回歸分析評估從不、偶爾、有時、大多時候、經(jīng)常攝入蔬菜或水果與高血壓之間的關(guān)系;關(guān)于分類蔬菜水果攝入量與高血壓之間關(guān)系的研究,數(shù)據(jù)來自美國國民健康和營養(yǎng)體格檢查2003-2006年9992個成年人個體,采用卡方檢驗以及控制性別的Mantel-Haenszel(M-H)卡方檢驗分析分類蔬菜水果和高血壓之間的關(guān)系。在Meta分析中,根據(jù)異質(zhì)性的大小采用固定或隨機效應(yīng)模型合并相對危險度及其95%置信區(qū)間。結(jié)果:在現(xiàn)況調(diào)查中,多因素logistic回歸結(jié)果顯示:與最低劑量的水果、蔬菜攝入量相比,最高劑量的水果(OR=0.50;95%CI:0.29-0.87;P=0.014)、蔬菜(OR=0.73;95%CI:0.54-0.99;P=0.044)攝入量與高血壓發(fā)病風(fēng)險之間顯著相關(guān)?刂菩詣e后的M-H卡方檢驗結(jié)果顯示:蘋果(OR=0.75;95%CI:0.63-0.89;P=0.001)、鳳梨(OR=0.82;95%CI:0.74-0.92;P=0.001)、生食的綠色蔬菜(OR=0.84;95%CI:0.76-0.92;P0.001)、黃瓜(OR=0.85;95%CI:0.75-0.96;P=0.007)與高血壓之間呈有統(tǒng)計學(xué)意義的負相關(guān)關(guān)聯(lián)。在Meta分析中,共納入包括本次現(xiàn)況調(diào)查在內(nèi)的26項研究,合并結(jié)果顯示:蔬菜水果(RR=0.81;95%CI:0.74 0.89;I2=84.7%;P for heterogeneity0.001)以及水果(RR=0.71;95%CI:0.60 0.84;I2=69.3%;P for heterogeneity=0.001)攝入量的增加可顯著降低高血壓的發(fā)病風(fēng)險,而蔬菜攝入量與高血壓發(fā)病風(fēng)險之間的關(guān)系無統(tǒng)計學(xué)意義(RR=0.90;95%CI:0.80 1.02;I2=54.1%;P for heterogeneity=0.042)。影響性分析結(jié)果顯示,沒有任意一項研究對蔬菜水果、水果、蔬菜攝入量與高血壓之間相對危險度的合并效應(yīng)值有統(tǒng)計學(xué)意義的影響。結(jié)論:水果攝入量的增加可能降低高血壓的發(fā)病風(fēng)險。在分類的蔬菜水果中,蘋果、鳳梨、生食的綠色蔬菜、黃瓜可能與高血壓發(fā)病風(fēng)險的降低相關(guān)。
[Abstract]:Objective: the relationship between the intake of fruits and vegetables and the risk of hypertension is unclear. The relationship between vegetable and / or fruit intake and hypertension was investigated by a situational survey and a Meta analysis. Methods: a study of the relationship between total vegetable and fruit intake and hypertension was conducted in the current survey, with data from 1, 200 adults for the National Health and Nutrition physical examination of the United States from 2007 to 2010. Univariate and multivariate logistic regression analysis was used to assess the relationship between never, occasionally, and most of the time, frequent intake of vegetables or fruits and hypertension; The study on the relationship between vegetable and fruit intake and hypertension was based on data from the United States National Health and Nutrition physical examination 2003-2006 for 9,992 adults. The relationship between vegetables and fruit and hypertension was analyzed by chi-square test and Mantel-Haenszel (M-H) chi-square test. In Meta analysis, the relative risk and its 95% confidence interval are combined with fixed or random effect models according to the size of heterogeneity. Results: in the current survey, the results of multivariate logistic regression showed that the highest dose of fruit (OR=0.50;95%CI:0.29-0.87;P=0.014) and vegetable (OR=0.73;) were higher than the lowest dose of fruit and vegetable intake. 95 CI: 0.54-0.99 P0. 044) intake was significantly associated with the risk of hypertension. In the Meta analysis, 26 studies, including the current survey, were included. The results of impact analysis showed that none of the studies had a significant effect on the combined effect value of the relative risk between vegetables and fruits, vegetable intake and hypertension. Conclusion: the increase of fruit intake may reduce the risk of hypertension. In classified vegetables and fruits, apples, pineapples, raw green vegetables, and cucumbers may be associated with reduced risk of hypertension.
