【摘要】：目的觀察人冠狀動脈粥樣硬化病灶結(jié)構(gòu)變化、CD40L、基質(zhì)金屬蛋白酶9(MMP-9)蛋白表達水平,探討CD40/CD40L信號在人冠狀動脈粥樣硬化發(fā)生發(fā)展中的作用。方法收集不同程度粥樣硬化的人冠狀動脈血管60例作為實驗組,以12例無病變的人冠狀動脈為對照組,常規(guī)HE染色觀察兩組人群冠狀動脈的組織結(jié)構(gòu)并用圖像分析軟件檢測病灶結(jié)構(gòu)的相關(guān)指標,免疫組織化學染色檢測CD40L、MMP-9蛋白表達水平,分析CD40L、MMP-9表達水平與動脈硬化病灶結(jié)構(gòu)變化之間的關(guān)系。結(jié)果發(fā)生粥樣硬化的冠狀動脈CD40L表達均增強,其表達主要分布于斑塊肩部和底部的泡沫細胞。同時病灶內(nèi)MMP-9表達水平增高并與CD40L水平、病灶大小尤其是壞死灶的大小呈正相關(guān),MMP-9表達水平與纖維帽厚度無明顯相關(guān)性。隨著MMP-9水平增高,纖維帽厚度/內(nèi)膜最大厚度比值降低。結(jié)論人冠狀動脈粥樣硬化病灶內(nèi)CD40L、MMP-9表達水平明顯增強,CD40L可能通過促進MMP-9表達,使血管病灶內(nèi)細胞外基質(zhì)分解加強,從而促進病變的進展及壞死灶的擴大,使粥樣病灶穩(wěn)定性下降。
[Abstract]:Objective to investigate the changes of lesion structure and the expression of CD40L, matrix metalloproteinase-9 (MMP-9) protein in human coronary atherosclerosis, and to explore the role of CD40/CD40L signal in the pathogenesis and development of human coronary atherosclerosis. Methods Sixty patients with different degree of atherosclerosis were selected as experimental group and 12 cases as control group. The tissue structure of coronary artery in both groups was observed by routine HE staining and the relative indexes of lesion structure were detected by image analysis software. The expression level of CD40L,MMP-9 protein was detected by immunohistochemical staining. To analyze the relationship between the expression of CD40L,MMP-9 and the structural changes of arteriosclerosis lesions. Results the expression of CD40L was increased in the coronary artery with atherosclerosis, and the expression was mainly distributed in foam cells in the shoulder and bottom of the plaque. At the same time, there was a positive correlation between the expression of MMP-9 and CD40L, the size of lesion, especially the size of necrotic foci, but there was no significant correlation between the expression of MMP-9 and the thickness of fibrous cap. With the increase of MMP-9 level, the ratio of fiber cap thickness to maximum intimal thickness decreased. Conclusion the expression of CD40L,MMP-9 in human coronary atherosclerotic lesions is significantly increased, and CD40L may promote the development of the lesions and the expansion of necrotic foci by promoting the expression of MMP-9 and enhancing the decomposition of extracellular matrix in the lesions. The stability of atherosclerotic lesion was decreased.
