不同形狀血管中醫(yī)用支架擴(kuò)張力學(xué)行為的研究進(jìn)展

【摘要】:血管支架介入術(shù)由于其微創(chuàng)性和高效性已經(jīng)成為目前治療由血管狹窄所引發(fā)的冠心病的主要治療手段,但支架內(nèi)再狹窄卻是影響支架遠(yuǎn)期效果的最大障礙,嚴(yán)重制約了其實(shí)際療效。首先從支架內(nèi)再狹窄的形成機(jī)制出發(fā),從固體力學(xué)角度回顧力學(xué)環(huán)境對(duì)再狹窄的影響,并主要闡述近年來支架在圓直、錐形、分叉、彎曲4種不同形狀血管內(nèi)擴(kuò)張的研究進(jìn)展,特別是從支架擴(kuò)張對(duì)血管應(yīng)力及血管形狀的改變與再狹窄的相關(guān)關(guān)系方面進(jìn)行綜述。最后,針對(duì)不同血管形狀對(duì)支架擴(kuò)張的影響,提出一個(gè)旨在提高支架擴(kuò)張性能及減少支架內(nèi)再狹窄的血管支架多目標(biāo)優(yōu)化設(shè)計(jì)方法。
[Abstract]:Because of its minimally invasive and high efficiency, stent intervention has become the main treatment for coronary heart disease caused by vascular stenosis, but stent restenosis is the biggest obstacle to the long-term effect of stent. It seriously restricts its actual curative effect. Starting from the formation mechanism of in-stent restenosis, this paper reviews the influence of mechanical environment on restenosis from the point of view of solid mechanics, and mainly describes the research progress of stent dilatation in four different shapes in recent years: straight, conical, bifurcation and bending. In particular, the relationship between stent dilation and vascular stress, shape and restenosis is reviewed. Finally, aiming at the influence of different vascular shapes on stent dilatation, a multi-objective optimization design method is proposed to improve stent dilatation performance and reduce restenosis in stent.
