血漿半乳糖凝集素-3水平與慢性心力衰竭的相關(guān)性研究

【摘要】:目的:通過監(jiān)測慢性心力衰竭(CHF)患者血漿半乳糖凝集素-3(Gal-3)及N末端B型利鈉肽原(NT-proBNP)的水平,探討血漿Gal-3水平在CHF中的臨床價(jià)值,進(jìn)而指導(dǎo)早期診斷CHF、評估病情嚴(yán)重程度及判斷預(yù)后。方法:選取CHF(紐約心臟協(xié)會心功能分級II-IV級)患者120例及對照組40例,抽血檢測所有入選研究對象的肝腎功能(AST、ALT、CR、BUN)、甘油三酯(TG)、總膽固醇(TC)、低密度脂蛋白(LDL-C)等常規(guī)生化指標(biāo)。酶聯(lián)免疫吸附法(ELISA)測定血漿Gal-3和NT-proBNP水平,超聲心動(dòng)圖檢查測定左室舒張末期內(nèi)徑(LVEDD)及左室射血分?jǐn)?shù)(LVEF),收集并詳細(xì)記錄所有研究對象的一般資料,包括年齡、性別、BMI、吸煙及飲酒史等,通過統(tǒng)計(jì)學(xué)分析及處理比較心衰組與非心衰組間差異。受試者工作特征曲線(ROC曲線)分析血漿Gal-3與NT-proBNP水平在CHF的診斷價(jià)值。結(jié)果:1.心衰組Gal-3水平顯著高于對照組(P0.05),且隨心功能分級的增加而升高;2.血漿Gal-3和NT-proBNP水平與LVEDD均呈正相關(guān)(r=0.766,P0.01;r=0.496,P0.05),與LVEF呈負(fù)相關(guān)(r=-0.634,P0.01);3.Gal-3對心力衰竭同樣有較高的診斷價(jià)值;4.血漿Gal-3水平在CHF不同病因組間無統(tǒng)計(jì)學(xué)差異。結(jié)論:血漿Gal-3水平與CHF具有一定相關(guān)性,血漿Gal-3水平與左室功能和結(jié)構(gòu)的變化相關(guān),表明血漿Gal-3參與了CHF左室重塑的過程,Gal-3對慢性心力衰竭有較高的診斷價(jià)值。因此可以指導(dǎo)早期診斷CHF、評估病情嚴(yán)重程度及判斷預(yù)后。
[Abstract]:Objective: to investigate the clinical value of plasma galactose agglutinin 3 (Gal-3) and N-terminal B-type natriuretic peptide (NT-proBNP) in patients with chronic heart failure (CHF) in CHF. Thus, early diagnosis and CHF, can be used to evaluate the severity of the disease and to judge the prognosis. Methods: a total of 120 patients with CHF (New York Heart Association Cardiac function Grade II-IV) and 40 controls were selected. The liver and kidney function (AST,ALT,CR,BUN), triglyceride (TG), total cholesterol (TC), low density lipoprotein (LDL-C) and other biochemical indexes were measured. The levels of plasma Gal-3 and NT-proBNP were measured by enzyme-linked immunosorbent assay (ELISA), left ventricular end-diastolic diameter (LVEDD) and left ventricular ejection fraction (LVEF),) were measured by echocardiography, and the general data, including age, were collected and recorded in detail. BMIs, smoking and drinking history were compared by statistical analysis and treatment between CHF group and non HF group. The diagnostic value of plasma Gal-3 and NT-proBNP in CHF was analyzed by ROC curve. The result is 1: 1. The level of Gal-3 in heart failure group was significantly higher than that in control group (P0.05), and increased with the increase of cardiac function grade. The plasma levels of Gal-3 and NT-proBNP were positively correlated with LVEDD (P 0.01) and negatively correlated with LVEF (r-0.634 鹵P0.01) 3.Gal-3 also had high diagnostic value for heart failure (P < 0.05). There was no significant difference in plasma Gal-3 levels among different etiological groups of CHF. Conclusion: plasma Gal-3 level is correlated with CHF, and plasma Gal-3 level is correlated with the changes of left ventricular function and structure, indicating that plasma Gal-3 is involved in the process of CHF left ventricular remodeling. Gal-3 has a high diagnostic value in chronic heart failure. Therefore, early diagnosis of CHF, can be used to evaluate the severity and prognosis of the disease.
