高血壓患者24 h尿鈉鉀比值對同型半胱氨酸水平的影響研究

【摘要】:目的探索高血壓患者24h尿鈉鉀比值對同型半胱氨酸(Hcy)水平的影響。方法選取2015年1月—2016年12月在新疆醫(yī)科大學第一附屬醫(yī)院心血管內科住院確診的高血壓患者958例為研究對象。根據病歷詳細收集患者一般資料。所有患者按照要求留取24h尿液標本,檢測24h尿鉀、尿鈉水平和尿鈉鉀比值,并在同一天抽取血液標本檢測血清鈉、血清鉀、Hcy、三酰甘油(TG)、總膽固醇(TC)等指標。根據所有入選患者24h尿鈉鉀比值水平按三分位法將其分為3組:尿鈉鉀比值≤4.05(T1組,n=320);4.06~6.16(T2組,n=319)和6.16(T3組,n=319)。采用多元線性回歸分析探討高血壓患者24h尿鈉鉀比值對Hcy水平的影響。結果 3組患者24h尿鈉水平為130~260 mmol/24h,中位水平為182.90(107.79)mmol/24h;24h尿鉀水平為25~100 mmol/24h,中位水平為35.73(18.37)mmol/24h;24h尿鈉鉀比值中位水平為4.99(3.33)。3組患者24h尿鈉、尿鉀、尿鈉鉀比值及Hcy水平比較,差異有統(tǒng)計學意義(P0.05)。多元線性回歸分析結果顯示,24h尿鈉鉀比值是Hcy的影響因素[b=0.409,95%CI(0.053,0.765),P0.05]?刂苹颊哐邂c鉀水平、性別、年齡、BMI、血壓等多個混雜因素后,24h尿鈉鉀比值仍是Hcy的影響因素[b=0.461,95%CI(0.063,0.859),P0.05]。結論高血壓患者24h尿鈉鉀比值是Hcy的影響因素。通過減少鈉攝入、增加鉀攝入等改善不良飲食習慣是高血壓非藥物治療的重要組成部分,應當引起廣大高血壓患者及臨床醫(yī)生的重視。
