厄貝沙坦聯(lián)合至靈菌絲膠囊對高血壓病患者微量白蛋白尿作用的臨床研究

【摘要】：目的:運用試紙法檢測高血壓病患者的尿微量白蛋白,探討門診就診的高血壓病患者微量白蛋白尿的陽性率,以及血管緊張素Ⅱ受體拮抗劑厄貝沙坦及中藥至靈菌絲膠囊對微量白蛋白尿的影響。方法:納入2015年12月～2017年2月在江蘇省中醫(yī)院心內(nèi)科門診及挹江門社區(qū)衛(wèi)生服務(wù)中心全科門診就診的已診斷為高血壓病的429名患者,運用研究用表收集相關(guān)研究信息,將納入患者按性別及年齡分組,進(jìn)行血壓特點分析比較;采集就診當(dāng)時新鮮尿液,應(yīng)用試紙法檢測患者尿微量白蛋白及肌酐水平,統(tǒng)計微量白蛋白尿(MAU)陽性率;篩查出MAU陽性的患者,以血管緊張素Ⅱ受體拮抗劑(ARB)為基礎(chǔ)降壓藥,連續(xù)治療1月后復(fù)測研究指標(biāo),進(jìn)行組內(nèi)治療前后MAU水平比較;MAU陽性患者隨機(jī)分為ARB聯(lián)合中藥至靈菌絲膠囊組及非聯(lián)合用藥組,觀察治療后MAU轉(zhuǎn)陰率。結(jié)果:1.男性患者的舒張壓顯著高于女性患者(p0.01);年齡越小,舒張壓越高;收縮壓組間無明顯差異。2.429名高血壓病患者M(jìn)AU陽性總檢出率為38.00%,男性患者尿微量白蛋白水平高于女性患者(P0.01)。3.ARB類藥物治療前后對比,治療后尿微量白蛋白水平顯著低于治療前(P0.01)。4.ARB聯(lián)合至靈菌絲膠囊治療后,MAU陽性轉(zhuǎn)陰率為69.77%,顯著高于對照組41.46%(P0.01)。結(jié)論:1.高血壓病患者M(jìn)AU總檢出率較高,男性患者、高齡者及青壯年患者的MAU陽性率較高,高血壓病管理過程中,應(yīng)重視對高齡患者及青壯年患者的MAU的檢測。2.ARB類藥物可以降低高血壓病患者的MAU陽性率及MAU水平,具有一定的早期腎損害防治作用。3.中藥至靈菌絲膠囊具有明顯的MAU陽性的轉(zhuǎn)陰作用,其作用機(jī)制值得進(jìn)一步探討。
[Abstract]:OBJECTIVE:To detect urinary microalbuminuria in patients with hypertension by test paper method,and to investigate the positive rate of microalbuminuria in outpatients with hypertension,and the effects of irbesartan,an angiotensin II receptor antagonist,and Zhiling mycelium Capsule on microalbuminuria.METHODS:The patients were enrolled in Jiangsu Province from December 2015 to February 2017. 429 patients diagnosed as hypertension in the outpatient department of Cardiology and the outpatient department of general health service center of Lijiang Men Community Health Service Center of Traditional Chinese Medicine Hospital were enrolled in this study. The positive rate of microalbuminuria (MAU) was calculated by measuring the levels of serum albumin and creatinine, and the patients with positive MAU were screened out. The patients with positive MAU were treated with angiotensin II receptor antagonist (ARB) as the basic antihypertensive drug for 1 month. The MAU levels were measured and compared before and after treatment. Results: 1. The diastolic blood pressure of male patients was significantly higher than that of female patients (p0.01); the diastolic blood pressure was higher with age; there was no significant difference between the systolic blood pressure groups. After ARB combined with Zhiling Mycelium Capsule, the positive rate of MAU was 69.77%, which was significantly higher than that of the control group (41.46%). Conclusion: 1. The total detection rate of MAU in hypertension patients was higher than that in male patients, elderly patients and young adults. In the process of hypertension management, attention should be paid to the detection of MAU in elderly patients and young adults. 2. ARB drugs can reduce the positive rate of MAU and the level of MAU in patients with hypertension, and have a certain role in the prevention and treatment of early renal damage. 3. Zhiling mycelium capsule has obvious negative effect of MAU, and its mechanism is worth further study. Discuss.
