【摘要】：目的探討膠原纖維在風(fēng)濕性心臟瓣膜病心房顫動(dòng)(房顫)患者中的異常表達(dá)以及與房顫發(fā)生的相關(guān)性。方法 33例風(fēng)濕性心臟病接受換瓣患者分為竇性心律組與房顫組。于手術(shù)中獲取右心耳組織。采用轉(zhuǎn)錄組學(xué)進(jìn)行差異表達(dá)基因的功能分析;RT-PCR檢測(cè)Ⅰ~Ⅳ型膠原纖維RNA在右心房組織中的表達(dá);免疫組化觀察心肌組織各型膠原纖維的表達(dá)與分布。結(jié)果與竇性心律組相比,房顫組左心房?jī)?nèi)徑明顯擴(kuò)大。轉(zhuǎn)錄組學(xué)分析顯示COL1A2、COL2A1、COL3A1以及COL4A3與COL5A2、COL6A3、COL11A1等膠原纖維存在相互關(guān)聯(lián)之外,與多種細(xì)胞因子也存在相互作用;RT-PCR結(jié)果示COL1A2、COL2A1、COL3A1和COL4A3均在房顫患者右心房組織中上調(diào)表達(dá),其中COL3A1、COL4A3上調(diào)表達(dá)顯著(P0.05)。免疫組化結(jié)果顯示Ⅰ型膠原纖維分布在血管內(nèi)皮細(xì)胞及血管基底膜;Ⅱ型膠原纖維分布在心肌細(xì)胞肌節(jié);Ⅲ型膠原纖維分布在間皮細(xì)胞及心肌細(xì)胞漿;Ⅳ型膠原纖維分布在心肌細(xì)胞外膜。結(jié)論心房組織中COL3A1與COL4A3表達(dá)異常,與多種細(xì)胞因子相互作用引發(fā)房顫的發(fā)生和發(fā)展。
