北京社區(qū)人群血壓與頸動脈內(nèi)中膜厚度的相關(guān)性探討

【摘要】:目的探討頸動脈內(nèi)中膜厚度增厚與否對血壓與頸動脈內(nèi)中膜厚度相關(guān)性的影響。方法研究人群來自北京大學(xué)第一醫(yī)院心內(nèi)科于2011年12月至2012年4月在北京市石景山區(qū)橫斷面調(diào)查的動脈粥樣硬化研究隊列。納入標準為問卷調(diào)查資料完整并完成頸動脈超聲圖像的采集且未應(yīng)用過任何降壓藥物,研究人群(n=2 569)以最大頸動脈內(nèi)中膜厚度0.9 mm為分界點分為頸動脈內(nèi)中膜厚度增厚組(n=1 597)及非增厚組(n=972),分析血壓與頸動脈內(nèi)中膜厚度的關(guān)系。結(jié)果多重線性回歸分析顯示,在調(diào)整了性別、年齡、BMI、目前吸煙飲酒狀況、血肌酐、空腹血糖、高密度脂蛋白膽固醇、低密度脂蛋白膽固醇、甘油三酯、降糖降脂藥物使用、心血管疾病患病情況等因素后,總體人群各項血壓指標(收縮壓、舒張壓、脈壓及平均動脈壓)均與頸動脈內(nèi)中膜厚度平均值呈顯著正相關(guān)(收縮壓:β=0.009,P0.000 1;舒張壓:β=0.002,P0.000 1;脈壓:β=0.007,P0.000 1;平均動脈壓:β=0.009,P0.000 1);在頸動脈內(nèi)中膜厚度≥0.9 mm組各項血壓指標依然與頸動脈內(nèi)中膜厚度平均值呈顯著正相關(guān)(收縮壓:β=0.006,P=0.000 1;舒張壓:β=0.001,P=0.000 1;脈壓:β=0.005,P0.0001;平均動脈壓:β=0.005,P=0.023 8),而非增厚組中各項血壓指標與頸動脈內(nèi)中膜厚度平均值均無明顯相關(guān)。結(jié)論本研究發(fā)現(xiàn)在頸動脈內(nèi)中膜厚度增厚的人群中血壓指標與頸動脈內(nèi)中膜厚度顯著相關(guān),非增厚人群中無顯著相關(guān)。提示血管壁增厚影響血壓的作用機制在二者相關(guān)性機制方面可能占據(jù)更重要的地位。
[Abstract]:Objective to investigate the effect of carotid intima-media thickness (IMT) on blood pressure and carotid intima-media thickness (IMT). Methods A cross-sectional investigation of atherosclerosis was conducted in Shijingshan District of Beijing from December 2011 to April 2012 in Department of Cardiology, first Hospital of Peking University. The inclusion criteria were complete questionnaire data, complete carotid ultrasound image acquisition and no antihypertensive drugs were used. The maximum carotid intima media thickness (0.9 mm) was divided into two groups: carotid intima-media thickness (IMT) group (n = 1597) and non-thickening group (n = 997 2). The relationship between blood pressure and carotid intima media thickness (IMT) was analyzed.
