【摘要】：目的探討舍曲林聯(lián)合心理干預(yù)對(duì)老年冠心病患者經(jīng)皮冠狀動(dòng)脈介入(PCI)后焦慮抑郁癥狀的改善情況及與血清白細(xì)胞介素(IL)-18、髓過(guò)氧化物酶(MPO)的相關(guān)性。方法 102例PCI手術(shù)伴有焦慮抑郁癥狀的冠心病患者102例隨機(jī)分為對(duì)照組54例,采用常規(guī)治療及給予安慰劑輔助治療,觀察組48例在對(duì)照組治療的基礎(chǔ)上給予鹽酸舍曲林片,并實(shí)施心理干預(yù)。結(jié)果治療后觀察組漢密頓焦慮量表(HAMA)評(píng)分、漢密頓抑郁量表(HAMD)評(píng)分均顯著低于對(duì)照組(均P0.05);觀察組血清IL-18、MPO水平明顯低于對(duì)照組(均P0.05);觀察組血清IL-18、MPO與HAMA評(píng)分呈正相關(guān)(均P0.001),血清IL-18、MPO與HAMD評(píng)分呈正相關(guān)(均P0.001)。結(jié)論舍曲林聯(lián)合心理干預(yù)治療可明顯改善介入治療冠心病患者術(shù)后焦慮抑郁癥狀,術(shù)后IL-18、MPO檢測(cè)可為患者的治療和預(yù)后研究提供靶點(diǎn)。
[Abstract]:Objective to investigate the effect of sertraline combined with psychological intervention on anxiety and depression after percutaneous coronary intervention (PCI) in elderly patients with coronary artery disease (CHD) and its correlation with interleukin-18 (IL-18) and myeloperoxidase (MPO). Methods 102 patients with coronary heart disease associated with anxiety and depression in PCI operation were randomly divided into control group (54 cases) and placebo adjuvant therapy. 48 cases in observation group were treated with sertraline hydrochloride on the basis of control group. Psychological intervention was carried out. Results the Hamilton anxiety scale (Hama) score in the observation group after treatment was higher than that in the control group. The scores of Hamd were significantly lower in the observation group than those in the control group (all P 0.05), the levels of IL-18 MPO in the observation group were significantly lower than those in the control group (all P 0.05), and there was a positive correlation between the serum IL-18 MPO and the Hama score in the observation group (P 0.001), and a positive correlation between the serum IL-18 MPO and the Hamd score in the observation group (all P 0.001). Conclusion Sertraline combined with psychological intervention can significantly improve anxiety and depression symptoms in patients with coronary heart disease after interventional therapy. The detection of IL-18 MPO can provide a target for the treatment and prognosis of patients with coronary heart disease.
