【摘要】：目的探索左后分支折返型室速患者在射頻消融術(shù)前的竇律體表心電圖特點(diǎn)。方法選取2007年1月-2017年1月在廣西醫(yī)科大學(xué)第一附屬醫(yī)院經(jīng)心臟電生理檢查確診左后分支折返型室速并行射頻消融術(shù)治療的患者共計45人。分別測量術(shù)前竇律時體表心電圖QRS波群的形態(tài)及額面心電軸,進(jìn)行統(tǒng)計分析。結(jié)果45例病人,術(shù)前竇律時QRS額面電軸中位數(shù)為+79°(-90°?+193°),電軸在正常范圍者23例(51.11%),電軸為+48°±35°,電軸左偏者2例(4.44%),電軸分別為-90°和-63°,電軸右偏者19例(42.22%),電軸為+107°±20°,電軸極度右偏者1例,電軸為+193°。術(shù)前呈現(xiàn)左后分支傳導(dǎo)阻滯者7人(15.56%),電軸在+93°?+133°之間;術(shù)前未呈現(xiàn)左后分支傳導(dǎo)阻滯者38人(84.44%),電軸在-90°?+86°之間。Ⅰ導(dǎo)聯(lián)QRS波呈現(xiàn)RS圖形者(即R波、S波振幅的絕對值相等)共9人(20%),電軸均為+90°。結(jié)論部分左后分支折返型室速患者竇律時的體表心電圖表現(xiàn)出左后分支傳導(dǎo)阻滯的特點(diǎn)。
[Abstract]:Objective to investigate the characteristics of sinus surface electrocardiogram in patients with left posterior branch reentrant ventricular tachycardia before radiofrequency ablation. Methods A total of 45 patients with left posterior reentrant ventricular tachycardia and radiofrequency ablation were selected from January 2007 to January 2017 in the first affiliated Hospital of Guangxi Medical University. The shape of QRS wave group and the frontal ECG axis were measured before sinus rhythm before operation, and the statistical analysis was carried out. Results 45 patients, Before sinus rhythm, the median of QRS frontal axis was 79 擄-90 擄-193 擄, 23 cases were in normal range, the axis was 48 擄鹵35 擄, the left axis was -90 擄and -63 擄, the right axis was -90 擄and -63 擄, the electric axis was 107 擄鹵20 擄, the electric axis was extremely right deviation in 1 case, and the electric axis was 193 擄. There were 7 patients with left posterior branch block before operation, and the electric axis was between 93 擄and 133 擄. The left posterior branch block was not seen in 38 patients before operation, and the electric axis was between -90 擄and 86 擄. The QRS wave in lead I showed RS pattern (i.e., the absolute value of amplitude of R wave S wave was equal). Conclusion the electrocardiogram of left posterior branch reentrant ventricular tachycardia showed the characteristics of left posterior branch block in sinus rhythm.
