【摘要】：背景吸煙是引起動(dòng)脈硬化的主要危險(xiǎn)因素,動(dòng)脈彈性減低是動(dòng)脈硬化的早期改變,分析吸煙及其相關(guān)危險(xiǎn)因素對(duì)動(dòng)脈彈性的影響有重要意義。血管回聲跟蹤(ET)技術(shù)可以在血管發(fā)生明顯的形態(tài)學(xué)改變之前檢測(cè)到動(dòng)脈彈性的減低。目的探討吸煙及其相關(guān)因素對(duì)中國男性單純吸煙人群頸總動(dòng)脈(CCA)彈性的影響。方法 2009年8月—2011年8月從全國各地46所醫(yī)院的國人吸煙人群調(diào)查數(shù)據(jù)中,選取符合納入標(biāo)準(zhǔn)的男性單純吸煙者915例為吸煙組。同時(shí)選取國人正常值手冊(cè)中的1 920例中國男性正常人群為正常組。記錄兩組年齡,吸煙組吸煙相關(guān)因素,即平均每日吸煙量、吸煙年數(shù)、開始吸煙年齡、尼古丁依賴程度評(píng)分。記錄吸煙組左右側(cè)CCA的內(nèi)膜中膜厚度(IMT),兩組CCA的彈性參數(shù)[硬度指數(shù)(β)、彈性系數(shù)(Ep)、順應(yīng)性(AC)、膨大指數(shù)(AI)及脈搏波傳導(dǎo)速度(PWVβ)]。結(jié)果吸煙組左、右側(cè)CCA的β、Ep、PWVβ均大于正常組,AC、AI均小于正常組(P0.05);吸煙組20~29歲、30~39歲、40~49歲、50~59歲、≥60歲左、右側(cè)CCA的β大于正常組(P0.05)。吸煙組10~19歲左側(cè)CCA,20~29歲、30~39歲、40~49歲、50~59歲、≥60歲左、右側(cè)CCA的Ep大于正常組(P0.05)。吸煙組20~29歲、≥60歲左、右側(cè)CCA,30~39歲左側(cè)CCA,50~59歲右側(cè)CCA的AC小于正常組(P0.05)。吸煙組10~19歲、20~29歲左、右側(cè)CCA,30~39歲、50~59歲右側(cè)CCA的AI小于正常組(P0.05)。吸煙組10~19歲、≥60歲右側(cè)CCA,20~29歲左、右側(cè)CCA,30~39歲、40~49歲左側(cè)CCA的PWVβ大于正常組(P0.05)。吸煙組平均每日吸煙量與CCA的IMT、β、Ep、PWVβ呈正相關(guān),與CCA的AC呈負(fù)相關(guān)(P0.05);吸煙年數(shù)、開始吸煙年齡、尼古丁依賴程度評(píng)分與CCA的IMT、β、Ep、AI、PWVβ呈正相關(guān),與CCA的AC呈負(fù)相關(guān)(P0.05)。多元逐步線性回歸分析結(jié)果顯示,平均每日吸煙量、吸煙年數(shù)是CCA的IMT的影響因素(P0.05);平均每日吸煙量、吸煙年數(shù)、尼古丁依賴程度評(píng)分是CCA的β、Ep、AC、PWVβ的影響因素(P0.05);吸煙年數(shù)是CCA的AI的影響因素(P0.05)。結(jié)論吸煙人群CCA彈性明顯減低,平均每日吸煙量、吸煙年數(shù)、尼古丁依賴程度的增加均是吸煙人群CCA彈性減低的危險(xiǎn)因素。
[Abstract]:Background smoking is the main risk factor for arteriosclerosis. The reduction of arterial elasticity is the early change of arteriosclerosis. It is important to analyze the influence of smoking and its related risk factors on arterial elasticity. Vascular echo tracking technique can detect decreased arterial elasticity before obvious morphologic changes occur. Objective to investigate the effect of smoking and its related factors on the elasticity of carotid artery (CCA) in Chinese male smokers alone. Methods from August 2009 to August 2011, 915 male smokers were selected as smoking group from 46 hospitals in China. At the same time, 1 920 Chinese male normal population were selected as normal group. Two groups of age, smoking related factors, namely, average daily smoking, number of years of smoking, age of starting smoking and nicotine dependence were recorded. The intima media thickness (IMT) of left and right CCA was recorded, and the elastic parameters of CCA [hardness index (尾), elastic coefficient (EPC), compliance index (AC), swelling index (AI) and pulse wave conduction velocity (PWV 尾)] were recorded in both groups.
