【摘要】：目的:探討風濕性心瓣膜病患者血漿內(nèi)脂素(Visfatin)、基質(zhì)金屬蛋白酶-9(MMP-9)水平與心肌重構(gòu)的相關(guān)性,以及內(nèi)脂素、MMP-9在圍術(shù)期的變化情況,為風心病雙瓣病變患者心肌重構(gòu)的評價提供更多依據(jù)。方法:選擇2014年6月至2015年10月我院胸心外科確診風濕性心臟瓣膜病擬接受雙瓣(二尖瓣及主動脈瓣)機械瓣置換術(shù)的患者38例,年齡45-55歲,心功能II~III級,同期選擇體檢健康成人12例設(shè)為對照組(C組)。排除標準:合并房顫、腫瘤、糖尿病、血液病、冠心病、嚴重肥胖(BMI≥28)者。實驗組根據(jù)術(shù)前心臟彩超結(jié)果分為:A組(左室舒張末期前后徑≥50mm),B組(左室舒張末期前后徑50mm)。實驗組分別于術(shù)前、術(shù)后48h、120h、240h采集肘部靜脈血,對照組采集清晨空腹靜脈血,測定內(nèi)脂素、MMP-9。實驗組于術(shù)中采集部分二尖瓣前乳頭肌標本,切片HE染色,做形態(tài)學觀察。結(jié)果:1)實驗組術(shù)前內(nèi)脂素、MMP-9較對照組增高(P0.01),其中,A組較B組更高(P0.01);2)實驗組中A、B兩組分別進行組內(nèi)比較,術(shù)后48h、120h、240h內(nèi)脂素、MMP-9較術(shù)前升高(P0.01),但術(shù)后48h、120h、240h之間差異無統(tǒng)計學意義;3)各時間點分別比較,A組內(nèi)脂素、MMP-9水平均較B組高;4)A組心肌細胞直徑以及心肌細胞間質(zhì)比例較B組高。結(jié)論:1)風濕性心瓣膜病患者血漿內(nèi)脂素、MMP-9水平較正常人明顯增高,且心室肌重構(gòu)程度越大其水平升高越明顯;2)風濕性心瓣膜病患者雙瓣機械瓣置換術(shù)后(手術(shù)48h后)內(nèi)脂素、MMP-9較術(shù)前升高,但手術(shù)近期(術(shù)后240h內(nèi))變化不大。
[Abstract]:Objective: to investigate the relationship between the plasma level of visfatinine, matrix metalloproteinase-9 (MMP-9) and myocardial remodeling in patients with rheumatic valvular heart disease, and the changes of MMP-9 in perioperative period. To provide more evidence for the evaluation of myocardial remodeling in patients with rheumatic heart disease. Methods: from June 2014 to October 2015, 38 patients with rheumatic valvular disease, aged 45-55 years, undergoing mechanical valve replacement with mitral valve and aortic valve, were enrolled in this study.
