【摘要】：目的探討美托洛爾與曲美他嗪聯(lián)合治療老年冠心病心力衰竭的臨床療效及對患者心功能、心肌重塑和炎癥因子的影響。方法將入選的176例老年冠心病心力衰竭患者根據(jù)收治順序單雙號分為觀察組和對照組,每組88例。兩組患者均給予常規(guī)對癥治療,在此基礎(chǔ)上,觀察組患者采用美托洛爾與曲美他嗪聯(lián)合治療。治療6個(gè)月后對比觀察兩組臨床療效、不良反應(yīng)以及生命體征、心功能、心肌重塑和炎癥因子等指標(biāo)變化情況。結(jié)果觀察組治療后總有效率(94.32%)顯著優(yōu)于對照組(85.23%)(P0.05);兩組患者治療后的HR、SBP、DBP、LVEDD、LVESD、IL-6、TNF-α和hs-CRP均較治療前顯著降低(P0.05),且觀察組降低更明顯(P0.05);與治療前比較,兩組治療后LVEF和6MWT顯著升高(P0.05),且觀察組治療后LVEF和6MWT顯著高于對照組治療后(P0.05);在治療期間兩組均未發(fā)生明顯不良反應(yīng)。結(jié)論美托洛爾聯(lián)合曲美他嗪對于老年冠心病心力衰竭患者治療效果優(yōu)良,能降低患者心肌重塑風(fēng)險(xiǎn),降低炎癥反應(yīng),有效改善患者生命體征、心功能,且安全性良好,值得臨床積極借鑒。
[Abstract]:Objective to investigate the effect of metoprolol combined with trimetazidine on heart function, myocardial remodeling and inflammatory factors in elderly patients with coronary heart disease. Methods 176 elderly patients with heart failure were divided into observation group (n = 88) and control group (n = 88). Two groups of patients were treated with routine therapy. On this basis, the patients in the observation group were treated with metoprolol combined with trimetazidine. After 6 months of treatment, the changes of clinical efficacy, adverse reactions, vital signs, cardiac function, myocardial remodeling and inflammatory factors were compared between the two groups. Results the total effective rate after treatment in the observation group was significantly higher than that in the control group, which was 85.230.The HRT SBP of the two groups after treatment was significantly lower than that of the control group, and the levels of IL-6, TNF- 偽 and hs-CRP in the two groups were significantly lower than those before the treatment, and the decrease in the observation group was more obvious than that in the control group, and compared with that before treatment, there was no significant difference between the two groups. LVEF and 6MWT increased significantly after treatment in both groups, and LVEF and 6MWT in the observation group were significantly higher than those in the control group after treatment, and no significant adverse reactions occurred in both groups during the treatment period. Conclusion Metoprolol combined with trimetazidine is effective in the treatment of elderly patients with heart failure. It can reduce the risk of myocardial remodeling, reduce inflammatory reaction, improve the vital signs and cardiac function of the patients, and the safety is good. It is worthy of clinical active reference.
