【摘要】：目的探討Z評分對評價正常中晚孕期胎兒主動脈(AO)、肺動脈(PA)內(nèi)徑增長的應(yīng)用價值。方法通過前后2次產(chǎn)前超聲心動圖測量332胎正常胎兒的AO內(nèi)徑及PA內(nèi)徑。以AO內(nèi)徑、PA內(nèi)徑為應(yīng)變量,胎兒雙頂徑(BPD)、胎兒股骨長徑(FL)、孕周(GA)為自變量,建立回歸方程,基于此計算AO、PA的Z評分。比較前后2次超聲心動圖檢查Z評分的差異。結(jié)果首次胎兒超聲心動圖檢查時GA為(25.82±2.27)周(18~35周),第2次檢查時GA為(30.61±2.28)周(23~39周)。前后2次胎兒超聲心動圖檢查所測胎兒AO內(nèi)徑、PA內(nèi)徑差異均有統(tǒng)計學(xué)意義(P均0.05)。2次超聲心動圖檢查的AO內(nèi)徑及PA內(nèi)徑均隨GA、BPD及FL的增長而增長。同一孕婦胎兒AO及PA的Z評分隨GA、BPD及FL的增長而相對穩(wěn)定。前后2次產(chǎn)前超聲心動圖檢查胎兒AO、PA的Z評分的差異均無統(tǒng)計學(xué)意義(P均0.05)。結(jié)論正常胎兒AO和PA內(nèi)徑的增長與生物學(xué)參數(shù)的增長呈正相關(guān),但Z-評分在中晚孕期相對保持恒定。
[Abstract]:Objective to evaluate the value of Z score in evaluating the diameter increase of AOA and PAA in fetal aorta and pulmonary artery during normal middle and late pregnancy. Methods AO and PA internal diameters of 332 normal fetuses were measured by antepartum echocardiography. The regression equation was established with AO internal diameter and PA internal diameter as dependent variable, fetal biparietal diameter as dependent variable, fetal femur long diameter flare and gestational weeks GA) as independent variables. The Z score of AOPA was calculated on the basis of this equation. The difference of Z score between two echocardiographic examinations was compared. Results GA was 25.82 鹵2.27 weeks in the first fetal echocardiography and 30.61 鹵2.28 weeks in the second check. There were significant differences in the internal diameter of AO and PA between the two fetal echocardiographic examinations (P < 0.05 or 0.05). The diameter of AO and PA increased with the increase of GABPD and FL. The Z scores of AO and PA in the same pregnant fetus were relatively stable with the increase of GAB PD and FL. There was no significant difference in Z score of AOPA between before and after two prenatal echocardiography (P < 0.05). Conclusion the increase of AO and PA in normal fetuses is positively correlated with the increase of biological parameters, but the Z- score remains relatively constant during the middle and late pregnancy.
