CT前瞻性心電門控技術(shù)對先天性心臟病診斷價值的Meta分析

【摘要】:目的:對CT前瞻性心電門控技術(shù)診斷先天性心臟病進(jìn)行Meta分析,評價在先天性心臟病的診斷價值。方法:檢索國內(nèi)外發(fā)表的CT前瞻性心電門控技術(shù)診斷先天性心臟病的文獻(xiàn)進(jìn)行Meta分析。提取資料,運用Meta-disc1.4軟件進(jìn)行異質(zhì)性檢驗,根據(jù)是否存在異質(zhì)性選用不同的效應(yīng)模型進(jìn)行合并。繪制森林圖,計算合并后的敏感度、特異度、似然比以及曲線下面積(area under curve,AUC)。納入的文獻(xiàn)的質(zhì)量評價根據(jù)Cochrane協(xié)作網(wǎng)的質(zhì)量評價工具(Quality Assessment for Diagnostic Accuracy Studies,QUADAS)進(jìn)行。結(jié)果:共12篇文獻(xiàn)被納入,總計1431處先天性心臟病畸形。CT前瞻性心電門控技術(shù)對先天性心臟病診斷靈敏度及特異度均不存在異質(zhì)性,采用固定效應(yīng)模型合并后的總敏感度為0.96(95%CI:0.94~0.97)、總特異度為1(95%CI:1.00~1.00)、陽性似然比為365.94(95%CI:231.18~579.26)、陰性似然比0.04(95%CI:0.03~0.05)。SROC曲線下面積為99.86%,Q*指數(shù)=0.9879。結(jié)論:CT前瞻性心電門控技術(shù)對診斷先天性心臟病診斷有較高的敏感度和特異度,同時,受試者工作特征曲線下面積較大,對于先天性心臟病具有較高的和穩(wěn)定的診斷性能。
[Abstract]:Objective: to evaluate the diagnostic value of CT prospective ECG gated technique in the diagnosis of congenital heart disease (CHD) by Meta analysis. Methods: Meta analysis was performed on CT prospective ECG gated diagnosis of congenital heart disease published at home and abroad. The data were extracted, the heterogeneity was tested by Meta-disc1.4 software, and different effect models were selected according to the existence of heterogeneity. The sensitivity, specificity, likelihood ratio and area under the curve were calculated. The quality evaluation of the included documents was conducted according to the quality Assessment for Diagnostic Accuracy study Quadas of the Cochrane collaboration Network. Results: a total of 12 articles were included and 1431 congenital heart malformations. Ct prospective ECG gating technique had no heterogeneity in the diagnostic sensitivity and specificity of congenital heart disease. The total sensitivity, specificity, positive likelihood ratio and negative likelihood ratio of the fixed effect model were 0.96 ~ 95 CI: 0.94 ~ 0.97, 1 ~ 95 CI: 1.00 ~ 1.00 ~ 1.00, 365.945.94 ~ 95CI: 231.181.181 ~ 579.26 and the area under the 0.04(95%CI:0.03~0.05).SROC curve was 99.86Q* ~ 0.9879, respectively. ConclusionThe sensitivity and specificity of the prospective ECG gating technique in the diagnosis of congenital heart disease are high. At the same time, the area under the operating characteristic curve of the subjects is larger, which has a high and stable diagnostic performance for the congenital heart disease.
