【摘要】：目的探討AngioJet血栓清除術(shù)在治療急性下肢動(dòng)脈缺血(ALI)中的臨床應(yīng)用價(jià)值。方法回顧性分析2015年1月至2016年11月經(jīng)AngioJet血栓清除術(shù)治療的12例ALI患者臨床資料,以心肌梗死溶栓(TIMI)治療后血流分級(jí)評(píng)估血流灌注情況,Cooley療效評(píng)分標(biāo)準(zhǔn)評(píng)價(jià)臨床療效。結(jié)果AngioJet血栓清除術(shù)技術(shù)成功率為91.7%(11/12),平均恢復(fù)灌注時(shí)間為(1.6±0.7)h。臨床成功率為83.3%(10/12),保肢率為91.7%(11/12)。TIMI分級(jí)由術(shù)前0級(jí)8例,1級(jí)4例,改善為術(shù)后即刻0級(jí)1例,1級(jí)3例,2級(jí)8例。11例(91.7%)下肢缺血癥狀有明顯改善,1例(8.3%)術(shù)后雖經(jīng)導(dǎo)管接觸溶栓(CDT)治療24h,但TIMI分級(jí)仍0級(jí)且肢體缺血癥狀未改善,予以外科膝上截肢治療。Cooley療效評(píng)分顯示,痊愈4例(33.3%),良好6例(50.0%),一般1例(8.3%),較差1例(8.3%),未發(fā)生嚴(yán)重出血并發(fā)癥。結(jié)論AngioJet血栓清除術(shù)可快速恢復(fù)ALI患者血流灌注,阻止病變進(jìn)一步惡化,提高保肢率,有較好的臨床應(yīng)用價(jià)值。
[Abstract]:Objective to evaluate the clinical value of AngioJet thrombus clearance in the treatment of acute lower extremity arterial ischemia. Methods the clinical data of 12 ALI patients treated with AngioJet thrombus clearance from January 2015 to November 2016 were retrospectively analyzed. Results the success rate of AngioJet thrombectomy was 91.7%, and the average recovery time was 1.6 鹵0.7h. The clinical success rate was 83.3% 10 / 12, and the limb salvage rate was 91.7%(11/12).TIMI grade 0 in 8 cases and grade 1 in 4 cases. The improvement was grade 0, grade 1, grade 2, 8 cases. 11 cases (91.7%). The symptoms of lower extremity ischemia were significantly improved in 1 case (8. 3%). Although the patients were treated with catheter contact thrombolytic therapy for 24 h, the TIMI grade was still grade 0 and the symptom of limb ischemia was not improved. Cooley score showed that 4 cases were cured with 33.3%, 6 cases were good, 1 case was 8. 3%, 1 case was poor, and there was no serious bleeding complication. Conclusion AngioJet thrombus clearance can quickly restore the blood flow perfusion of ALI patients, prevent further deterioration of the lesion, and improve the limb salvage rate, which has a good clinical application value.
