【摘要】：目的探討厄貝沙坦對(duì)老年H型高血壓合并糖尿病患者靶器官的影響。方法選擇2014年6月至2016年1月在鄭州大學(xué)附屬南陽市中心醫(yī)院接受治療的124例H型高血壓合并糖尿病患者,按照隨機(jī)數(shù)字表法將其分為觀察組與對(duì)照組,各62例。對(duì)照組予以非洛地平緩釋片治療,觀察組則予以厄貝沙坦治療,比較兩組患者治療前與治療1年后血壓、血糖、靶器官損傷指標(biāo)及靶器官損傷情況。結(jié)果經(jīng)相應(yīng)的治療后,兩組患者血壓、血糖、血脂等指標(biāo)均有不同程度的改善,且觀察組收縮壓變異性、舒張壓變異性及Hcy水平明顯低于對(duì)照組,高密度脂蛋白膽固醇(HDL-C)水平明顯高于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05);觀察組左心室質(zhì)量指數(shù)(LVMI)、尿微量清蛋白/肌酐比值(ACR)較對(duì)照組明顯降低,內(nèi)生肌酐清除率(Ccr)較對(duì)照組明顯升高,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組左心室肥厚、腎損傷、腦損傷的發(fā)生率及存在靶器官損傷的患者百分比均低于對(duì)照組,差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論厄貝沙坦對(duì)老年H型高血壓合并糖尿病患者的靶器官改善作用較明顯,且并發(fā)癥較少。
[Abstract]:Objective to investigate the effect of irbesartan on target organs in elderly patients with H-type hypertension and diabetes mellitus. Methods 124 patients with type H hypertension complicated with diabetes were treated in Nanyang Central Hospital affiliated to Zhengzhou University from June 2014 to January 2016. The patients were divided into two groups according to the method of random number table: the observation group and the control group, 62 cases each. The control group was treated with felodipine sustained release tablet, the observation group was treated with irbesartan. The blood pressure, blood glucose, target organ damage index and target organ injury were compared between the two groups before and after treatment for one year. Results the blood pressure, blood glucose, blood lipid and other indexes were improved in the two groups, and the systolic pressure variability, diastolic blood pressure variability and Hcy level in the observation group were significantly lower than those in the control group. High density lipoprotein cholesterol (HDL-C) level was significantly higher than that in the control group (P 0.05), and the left ventricular mass index (LVMIN) and urinary albumin / creatinine ratio (ACRs) in the observation group were significantly lower than those in the control group. The creatinine clearance rate (CcrR) was significantly higher than that in the control group (P 0.05). The incidence of left ventricular hypertrophy, renal injury, brain injury and the percentage of patients with target organ injury in the observation group were lower than those in the control group (P 0.05). Conclusion Irbesartan can improve the target organ of elderly patients with H-type hypertension and diabetes mellitus with less complications.
