不同族群CYP2C19基因多態(tài)性與凝血功能相關(guān)性分析

【摘要】:目的:通過(guò)確定CYP2C19基因型及在維吾爾族、哈薩克族、漢族人群中的等位基因分布特征,明確基因多態(tài)性與民族差異性之間的關(guān)系,根據(jù)凝血功能檢驗(yàn)結(jié)果的不同,得出不同CYP2C19等位基因?qū)β冗粮窭着R床療效的關(guān)系,從而達(dá)到協(xié)助調(diào)整用藥劑量,提高藥物療效,減少藥物毒副作用的目的。方法:選取新疆醫(yī)科大學(xué)一附院干內(nèi)二科住院冠心病患者520例,采用聚合酶鏈-限制性片段長(zhǎng)度多態(tài)性方法分析CYP2C19基因多態(tài)性,了解基因型分布在3個(gè)民族間的差異。同時(shí)從中選取77例無(wú)親緣關(guān)系的維吾爾族冠心病患者,均給予75mg氯吡格雷常規(guī)劑量每日一次口服,一定時(shí)間后觀(guān)察其靜脈血纖維蛋白原、凝血酶原時(shí)間(PT)、部分凝血酶原時(shí)間(APTT)結(jié)果。根據(jù)凝血功能檢驗(yàn)結(jié)果的不同,得出不同CYP2C19等位基因與氯吡格雷臨床療效對(duì)凝血功能影響的關(guān)系。結(jié)果:檢測(cè)到CYP2C19的6個(gè)基因型在維吾爾族、哈薩克族、漢族間分布差異顯著(P0.05)。同時(shí)根據(jù)對(duì)77例維吾爾族患者等位基因不同進(jìn)行分組。然而CYP2C19*1,CYP2C19*2,CYP2C19*3各組之間纖維蛋白原、凝血酶原時(shí)間、部分凝血酶原時(shí)間之間無(wú)明顯差異性,無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論:在維吾爾族、哈薩克族、漢族人群中,CYP2C19的基因型分布有顯著差異,可指導(dǎo)臨床個(gè)體化用藥。通過(guò)對(duì)77例維吾爾族冠心病患者的凝血功能指標(biāo)檢測(cè)發(fā)現(xiàn)等位基因*2、等位基因*3對(duì)凝血功能的影響無(wú)明顯差異性,無(wú)統(tǒng)計(jì)學(xué)意義。
[Abstract]:Objective: to determine the relationship between genetic polymorphisms and ethnic differences by determining the genotypes of CYP2C19 and the distribution of alleles in Uygur, Kazak and Han populations, according to the different results of coagulation function test.The relationship between the clinical efficacy of clopidogrel with different CYP2C19 alleles was obtained, so as to help adjust the dosage, improve the efficacy and reduce the side effects of clopidogrel.Methods: the polymorphism of CYP2C19 gene was analyzed by polymerase chain-restriction fragment length polymorphism (PCR) in 520 patients with coronary heart disease (CHD) in the first affiliated Hospital of Xinjiang Medical University.77 unrelated Uygur patients with coronary heart disease were given 75mg clopidogrel once a day, and their venous fibrinogen was observed after a certain time.Prothrombin time (PTT) and partial prothrombin time (APTT).According to the different results of clotting function test, the relationship between different CYP2C19 alleles and clopidogrel clinical effect on coagulation function was obtained.Results: six genotypes of CYP2C19 were detected in Uygur, Kazak and Han nationality.At the same time, the alleles of 77 Uygur patients were divided into groups.However, there was no significant difference in fibrinogen, prothrombin time and partial prothrombin time between the three groups.Conclusion: the genotype distribution of CYP2C19 in Uygur, Kazak and Han populations is significantly different, and it can be used as a guide for clinical individualized drug use.The coagulation function indexes of 77 patients with coronary heart disease in Uygur nationality were detected. The results showed that allele 2 and allele 3 had no significant difference in coagulation function and had no statistical significance.
